So if I understand it right, the rat had the parasite in him even before the transplant, the human tried to save him, and sense he didn't get the transplant and even with the transplant he dies at the end of the day so no matter what the rat dies at the end of the day. Oof.

I read somewhere that the rat had a heart disease by default, hence why he feels weak and “heavy” before the transplant, and why he told Heart he was going to die whether the surgery happened or not, while also explaining why the human specifically chose Mad Rat for the heart transplant instead of any of the other rats inside the cage, while also using them as test subjects for some inter-species transplanting experiment, that if successful, it could’ve been a huge discovery that could save many many lives. Also the fact that after the transplant, Mad Rat meets Rat God for the first time, that could mean that Heart was the one carrying the parasite, and Heart passed it to Mad Rat when he became his new heart, and may also explain how the parasite knows how to control Heart’s time powers, as it’s been with him and many other cats since who knows how long, and the moment the parasite acknowledged that it‘s inside a dying rat, it had to act immediately or it would perish as well

The only example I know off the top of my head is that pig skin can be grafted onto a human, usually a burn victim, and skin is technically an organ. I would assume immunosuppressants are involved and the pig skin is eventually "phased out" as new skin grows underneath, cuz normally transplants are an awful mess.

No, because of science, he had a heart transplant. But cats usually carry a specific parasite called Toxoplasmosis and in this game, that's Rat God. After tje transplant, Mad Rta was also effected and then all the shit happened.

There's another issue. If the heart came from a cat I have no knowledge of it being common practice to euthanize one animal to save another if that animal seems perfectly healthy. THis leads me to believe the even darker possibility that the cat was going to die anyway too, but wanted to die along side the rat. WHich means that the cat was dying of something else, but still had her healthy heart to give (my theory being a stunted heart, as the heart was about the size of the rats which means it wasn't enough to properly fulfill the needs of her body, but it was for him). Which makes me think in the end when she came back for him, it was so that the two of them could potentially die together some place they love instead of alone.
