no, it is not tilt or trim, it is a view inside the engine on the crankshaft, and the shaft is moved a little back and forth.

The stern tube has its own bearing and seals.

What prevents the sea water coming back into the ship through where the shaft exits the hull to join the propeller?

yes but mostly on newer cruise ships not on cargo and tankers most of the time because of cost and other factors
