That is because this class of train exists quite literally everywhere in the uk. That aside, the fisherton tunnel crash was caused by lack of rail adhesion because of track condition, not anything to do with 158s specifically.

And the same issue I understand, you can't stop if your wheels won't grip the rails. One slight bit of luck is that the modern signalling on the line stopped the train going the other way when the system knew it wasn't stopping, meaning the closing speed was much lower.

Apparently one of the two-carriage trains had entered the diversion-loop as it was supposed to but had then returned to the main-track instead of halting before the end of the loop. However, I'll leave it there until the investigators reveal exactly what happened!

Anyone can die from hitting their head. It doesn't take much to cause someone to fall during a collision. Alternatively, a piece of luggage could have fallen from the overhead bins. As the officer said, it's yet to be determined.
