I think it may have been that he triggered the criteria for both killing your stall neighbor with the grenade *and* for killing himself with the grenade, with the -1 Ending being a sort of "safety mechanism" so the game doesn't crash if you fail to meet the criteria for any of the other endings, or meet the criteria for 2 or more endings.

You’re missing endings 5 and 12, I suggest keep knocking on the stall door because it started to get dark, or stay in that stall in the ladies room because it started to get dark. Also try bringing the headband on the floor to the guy in the stall.

8:10 Usually in programming, we use negative numbers where there shouldn't be negative numbers to indicate an error or unexpected path, usually -1 is used as a generic "all other errors" rather than a specific error, in high production stuff (software that will effect at least tens of thousands of lives in some way) this is rarely seen as it provides no information as to what the problem is, but in lower production stuff (such as something only you or a few people will use...or an indie game) this is seen more often as it is the lazy approach that is also appropriate when you don't expect other people to be debugging your software.
