From 1980 through 2022 my platelets averaged 466,000, my HgB averaged 15.7, and my HCT averaged 46.4 with a JAK2 mutation confirmed. BMB confirmed signs of ET and nothing else. So I've been classified as asymptomatic ET, low risk. As a now 60 year old man, my new hemo is thinking I actually have PV, not ET because my HCT is always so high (last three were 49, 45.3, 48.1 and 46.9). I guess I am a blade runner -- just under the new proposed criteria in this video. Although the treatment would not be different either way apparently: 1 daily baby aspirin and once I'm ready, starting with daily HU of 500 mg.

I was just diagnosed with PV, my JAK2 came back negative but my CBCs have been off for more than 10 years now and my symptoms have been progressive, so my hematologist pushed for the bone marrow, and sure enough it was positive for PV, which is what we thought it was, to begin with, go figure...

what about those triple negative jak2 exlon12 Aclr9 with both hgb and hct high with low epo.

It depends on how high hgb and hct are and how low epo is. If they continue to change overtime, repeat the molecular tests. You need a considerable number of cells to get a positive gene test. The disease progresses slowly so it could take some time for genetic tests to be positive.
