Dude, you and your channel are a god send man. I have watched literally every single video here and that too multiple times. I got my friends watching this channel. Hell, even my rents know about this damn channel and they're not even in medicine. You have something beautiful man. I honestly can't wait till a complete dirty USMLE full study guide comes to fruition and people say "yeah I'm studying UFAP-DIRTY." I hope you make an insane amount of money. I am about to be (hopefully) a non-US img. Everyone tells me I'm a fool for even thinking about giving the USMLE, let alone applying to surgery(I wanna go for neurosurgery or plastic) Your video definitely showed me that those people were right. Someone like me has to get 20-30 percentile score than a US graduate with at least 15 publications. I don't even have one. Should I cut my losses now and aim for PLAB or Australia. I am from Pakistan and people here already have a really hard time getting a visa to USA. Add everything else on top and should I just abandon my dreams? I know people from Pakistan who did go abroad but they seem to be decrease in no. each year.

the realistic truth is you'll never match Plastics/NS as a non-US IMG. I'm just being realistic and I don't mean to discourage you. You should choose a different specialty if your goal is to match in the US

@@DirtyMedicine Can I first try to match in general surgery and then later try to subspecialise in one of these specialties. Is that allowed in the US?

@@HT.100 You can do that, there are several non-US IMGs that take the independent plastic surgery route. It's not that you'll never match plastics or Neurosurg, but its 99% unlikely that you will based off stats. There are a couple non-US IMGs in integrated plastic surgery (where you match straight into plastics), but these guys did 2-3 years of Plastic surgery specific research at an institution associated with a plastics residency.

HT my classmate was an img who did his neurosurgery after toiling in general surgery. It’s doable but takes time, good contacts, solid credentiAls, and a lot of patience. He just graduated last year at 40

Regarding research, does it have to be surgical research? In my country, research in surgery is very hard to come by so I have been publishing articles on cardiology and pneumology (since I want to match to general surgery and hopefully then get into a fellowship on cardiothoracic surgery ) Thanks!

This may seem like a silly question but in relation to IMGS, when they speak of work experience and volunteer programs could you give possible examples or suggestions of what I could do. Please help. I will be graduating in 2022 and would want to start my preparation now as I know it’s going to be a long journey. As of now I’ve done nothing in terms of research, work etc so I’m starting from scratch.

Work experience means actual employment. Research includes things like published case reports, bench research, participation in a true project who’s only goal was research.

The hours and pay for general surgery are terrible. You'll be spending your entire career taking out gallbladders and appendixes. I think you did a great job talking about the initial appeal of surgery in med school ("everyone wants to be a surgeon") but most people drop it once they rotate through the surgery ward during year 3.

Objectively speaking, the adjusted pay per hour worked is on the lower end for all specialties. However, the absolute value of the salary that surgeons earn is still quite high.

That is for people who REMAIN in general surgery. Increasingly, more and more people continue on to fellowship after general surgery residency, doing things like surg onc, HPB, pediatric, colorectal, MIS, transplant, endocrine, trauma/acs. All of these require you to be more academic, have slightly better lifestyles (except transplant) and better pay.
