Urology is one of the toughest to get in. You will need dedicated research for a substantial period of time here in the US. But based on home country residency in urology, other specialties become much easier. Please start with nrmp charting the match report

Home country research is valuable but in most cases US research is more valued. Also in terms of LORs 3 US lors are good. You are allowed 4.

Applying for observerships (and if you don't have any US LORs then go for home country LOR if they need it). For the ERAS application it depends on your profile. If you are a recent graduate then yes you can use one home country LOR. some programs may want an LOR from your school. Else best is to get LORS from US for the ERAS application.

It is not that old yog cant match. But if you are asking when programs consider you older yog that is mostly after 5 years of graduation.
