Subaru in the beginning mentally decided to nope the fuck out after the last loop. having tried hard and come up empty his mind decided "fuck it i'll just act like an invalid and let everyone else shoulder the responsibilities and hopefully it will end better than these". His "insanity" was "sane" as it had a goal, to seek sympathy from those around him who would have to deal with it, that is why he responded to the outer world and people around him. Sloth saw this and being someone who really is insane pointed out that if he was really insane he wouldn't give a damn about anything outside. Sloth could recognize this because he is fucking insane. At the end of this episode, Subaru has a form of insanity, blind rage.
