@@veanne Hello Veanne, it helps to write down all the things you need to complete each skill and then go over in your head what needs to be done after watching the video a few times and know what needs to be done it helps you know what you need to work on. Also break down the skills down in the 3 different parts this helped me because it seems overwhelming at first too because I was studying on my own as a challenger. You have First (Handwashing) then the other Skills are broken down like this Written Skills: where you have to document. 1) Pulse 2) Empty Unrinary Drainage Bag 3) Feeding 4) Respiration Activity of Daily Living Skills (ADL): 5) Mouth Care 6) Denture Care 7) Hand and Nail Care 8) Foot Care 9) Partial Bedbath 10) Perineal Care 11) Catheter Care 12) Bedpan 13) Make an Occupied Bed 14) Dress Patient with Weak Arm Mobility Skills: 15) Range of Motion (Shoulder) ROM. 16) ROM (Elbow and Wrist) 17) ROM (Hip, Knee and Ankle) 18) Transfer from bed to wheelchair 19) Ambulate with Gait Belt 20) change position to supported side lying. You don't multiply it by anything. You count the pulse for a full minute and then write that number down.

The beats you counted within your first 15 seconds is your accurate Pulse, calculate the number of beats you had with in your first 15 seconds and multiply it by 4.

in my program (in FL.) I was instructed to record the second pulse and then wash my hands, not the other way around. Does it matter if we record the second pulse first and then wash our hands or do we have to wash our hands before recording the second pulse?

For the exam, this is not a graded checkpoint, because you have to count twice so you will have to document the first reading before doing the second one (no handwashing required). However, in a clinical setting, it is ALWAYS a good practice to wash your hands before documenting, because you don't want to contaminate your pen with patient germs - your pen goes home with you. For the test, the person playing the patient will be following a script, so this will not be a concern for the test. In a clinical setting, follow your facility policy and procedures. Best of luck!

You are not counting the seconds on the clock. You are counting the thumps you feel under your fingers during this minute. This is the pulse. It feels like thumps, or throbs, under your fingers. Since this is a video, you cannot feel what I am feeling as I count, but I am counting the number of thumps I am feeling. You must count for one full minute, because the care plan tells us to. This video merely shows you HOW to take the pulse, you must do it yourself to actually feel what I am feeling when counting. Hope this helps! :-)

Is it that the gloves is not involved in the skills when taking the pulse?

No, gloves are not required for this skill. Gloves are only required when touching body fluids, personal skin or non-intact skin. You should not wear gloves when they are not required. Knowledge of infection control principles is important in healthcare. Best of luck!

So after were done the first time, should I still open the curtain, give the call light etc or not since we have to do it a second time? And if we do the whole process, do I repeat every single step for the 2nd time?

No, during the exam, you will not have to do the opening and closing twice. You will do the opening, wash your hands, count the pulse with the 1st evaluator, document the reading, count the pulse with the second evaluator, do your closing, wash your hands and then document the second reading. For the test, when the nurses switch to count the second pulse, am I allowed to document the first pulse right after I take it? My teacher said that if I were to do so, that the pen is considered contaminated and I'd have to wash my hands again. Also at the end of the Skill when we open the curtain, can we document then wash our hands? I thought hand washing was the absolute last thing you must do

For the exam, you are permitted to write down the pulse after taking it with the first evaluator. Documentation should USUALLY be done after handwashing to prevent transfer of germs onto your pen (which goes into other patient environments and home with you) but the test does not expect you to remember 2 pulse readings. Just for clarification, we can write down the first pulse to document but with the second nurse we should wait until the skill is finished. Once we open the curtain and wash our hands then we should document the second pulse finding. Is that correct?

Correct! Counting twice is specific to the test and won't be done in a clinical setting, so they make testing allowances for this. They don't expect you to remember the first reading while you are counting the second, so write down the first reading and then pick up from there as if it were your first reading, if that makes sense? Opening, (first reading, document), count pulse, closing, document. Hope this helps!

All pulse rates are measured per minute. If you are counting for 15 seconds and multiplying by 4, that gives you a count over one minute. Or, as this care plan instructs, if you are counting for one whole minute, it still gives you a measurement per minute. the per minute is implied in healthcare settings because that is universally accepted that all pulse rates are reported per minute. So, in a clinical setting, you may have a place to document pulse readings that does not indicate per minute because all pulse rates are SUPPOSED to be documented as per minute. Hope this helps! You will not be allowed to use a watch during the exam because smart watches can be used to cheat (and smart watches can look like plain watches). There will be a clock right above the bed for testing, just like in this video. Use the clock that is provided to time this skill. Best of luck!

You must announce when you start counting and when you stop counting so the evaluator can also count during this time to gauge your accuracy and ensure that you have counted for one full minute. In most testing facilities, you are not permitted to wear a watch due to smart watches being able to look like a regular watch (and smart watches are an electronic device that can allow the testing student to cheat). I'm sorry, but I am not entirely sure what your question is. Yes, when counting the pulse, you will count the thumps that you feel under your fingers, starting with the number 1 and counting upward with each thump. You must count the thumps you feel for one full minute. Hope this helps!

Ada John that happen to me when I took my clinical skill test! I lost count and the pulse and it screwed me up and I ended up failing that portion because I didn’t know for sure if I was able to tell the NAE!

In florida they have 2 evaluators and both listen with you one at a time to ensure accuracy. Fortunatly we only have one evaluator in nevada

For my CNA can leave the patient hand flat on the bed by placing fingertips on the thumb side of the wrist?

Yes, you can leave the hand flat on the bed...but then you would probably have to bend and that might not be comfortable. Keeping the elbow on the bed and raising the forearm places the radial pulse within reach without having the CNA in a stooped position. Hope this helps! Thank you for your comment! Reason why I ask is because when I try to find the pusle with raisin the forearm its hard for me to find it but when I try with the hand flat using my two finger tips i find it quickly. I don't know what am doing wrong tho.

It's probably the position of the tendon over the artery when the hand is in the upright position. You can try rotating the hand slightly in either direction - that may help. It is certainly okay to leave the hand lying flat on the bed, if that is easier! Either way is fine. Best of luck!

she is doing it wrong... nurse is not supposed to hold the hand when doing pulse. patient's hand must be laying flat on the bed.

There is no requirement for the hand to be laying flat on the bed, and raising the forearm will have no effect on obtaining the pulse. The ELBOW must be supported at all times, but you may raise the forearm up as long as the elbow remains in the bed. This skill confirms to the clinical skills checklist used on the state exam. I was wondering the same thing, I failed this skill on my test. I was trying to figure out what i did wrong. Maybe I was just off on my count.

Did you count the radial pulse (measured at the thumb side of the wrist for ONE FULL MINUTE? This is the reason most people fail this skill - they do not count for one full minute. The care plan is very explicit in this. If you count for any other amount of time, you are not following the care plan and therefore, will fail. Best of luck!
