Are we allowed to raise HOB up and adjust patient pillow without washing our hands at the begining of skill? I thought we must wash our hands before touching patient or patient's pillow.

Either way is appropriate. If adjusting the head of the bed after washing your hands, use a paper towel between your hands and the controller, so you don't contaminate your hands before touching the clean supplies. Hope this helps!

Either, depending on the water quality, facility resources and layout. If the water quality is good, and within reasonable distance, and the facility does not provide bottled water for patient use, you will be instructed to use the tap water. If the tap water quality is NOT good, or it is too far away, or the facility stocks bottled water for patient use, you will be instructed to use the bottled water. Always follow the facility policy. Question: My states CNA exam is pometric, isn't any fluids suppose to be discarded in the toilet? Are these ways performed also pometrics way?

If it NORMALLY goes in the toilet, then that's where it will go. So, urine, feces, emesis will all go in the toilet.But you brush your teeth over a sink, so saliva would go in a sink after mouth care. Bathing water though does not HAVE to go in the toilet, since you don't wash in a toilet. For peri-care and catheter care, soiled washcloths are not put back in the basin, so that water is not "contaminated" with body fluids that have to go in the toilet. This isn't a graded checkpoint, so feel free to put body fluids anywhere you want. Thanks for the comment!

I'm living in Michigan I was told to mixed 50 percent of water and 50 percent of mouth wash , using a oral swab to swab the mouth then we begin brushing of the teeth .

For the exam, you MUST follow the care plan you are given. Do not alter the care plan in any way. The care plan for the test (displayed at the beginning of the video) does not say anything about swabbing the patient's mouth with mouthwash and an oral swab. By adding a step not listed on the care plan, you are changing the care provided to the patient - CNAs are not qualified to make those decisions. Simply follow the care plan, the whole care plan and nothing but the care plan. :-) Good luck!

for the test would you need to raise the bed for working height or is raising the HOB from 60-90 degrees enough for working height ?

It depends on what state you are testing in. In Florida (and many other states), you are not graded on raising the bed to a comfortable working height because not all beds in all settings have that capability. But some states, like Kentucky, still require this step. Raising the head of the bed is different from raising the entire bed. Raising the entire bed makes it easier to reach the patient, requiring less bending for the care giver. But raising the head of the bed for this skill is all about safety and is an essential setp! You cannot brush someone's teeth when they are lying down because it is a choking risk. Therefore, you MUST raise the head of the bed to ensure patient safety. Failure to do so may result in a failing grade, because you are putting the patient at risk. So the HOB has nothing to do with YOUR comfort, it has everything to do with PATIENT safety. Q: at 2:31 it looks like you are brushing the back of the front teeth. Can you clarify should I announce to the instructor that I am brushing the backside of the front of teeth?

Yes, you are brushing whatever the patient has in their mouth (as you would brushing your own teeth, or your child's teeth). You don't have to announce it, they will see what you are doing. For testing purposes, the basin must be rinsed, dried and stored. But you cannot allow your dirty gloves to touch the basin after rinsing. The test used to require disinfecting the basin (which is why it is set down - it would be sprayed with disinfectant while resting in the sink, then picked up with a paper towel and the outside sprayed, then dried) but we are no longer able to disinfect during the exam. Therefore, we rinse, set down, pick up with a paper towel, dry and store the basin according to testing standards. I agree, the sink is not "clean" and I would prefer to disinfect, but that is no longer allowed. Hope this helps!

i have a question. when providing mouth care to a comatose resident, does the resident lay flat and turned onto his side to prevent choking or aspriation? Or does the resident lay in a partial upright postion and turned onto his side to prevent choking? retesting soon and would like to know which teqnique is correct for the state exam.

You would use whatever position the care plan indicates. Since we don't know if the patient has a spinal injury, recent abdominal surgery, musculoskeletal diseases, breathing problems, dysphagia, paralysis or other condition that would preclude one of these positions, it is impossible for me to tell you definitively which position is best for this task. In a clinical setting, the nurse will consider all this information and make the best possible informed decision and indicate the appropriate position on the care plan for the entire team to follow. This is NOT a skill on the state exam that you will be asked to demonstrate. However, (theoretically) if it were, the care plan would have to tell you what position the patient should be in since the patient is unable to respond to commands. Hope this helps and best of luck!

It depends. The evaluator will let you know if they need to see a correction. For instance, saying "I forgot to close the privacy curtain at the beginning of the skill" is fine and the evaluator will not need to see it. But saying "I would have brushed her teeth" is not. This action needs to be viewed. Always follow the direction of the evaluator. Hope this helps.

For the exam, yes. Please do not put a used toothbrush back into the drawer for the test - this can contaminate testing supplies. In a clinical setting, you can rinse and store the toothbrush for daily use (just like you rinse your own toothbrush daily). The patient will not use a new toothbrush every day (that can get expensive!) so simply store it in the top drawer with the other grooming supplies or in a toothbrush holder in their bathroom (per their preference). But the patient for the test is just an actor and for the test the toothbrush should be thrown away. Thank you for your comment!

The outside of ANY basin in not considered clean, in any setting. This is a general infection control principle. This procedure allows for the disinfection process, which is not performed during testing. Please see our video on basin cleaning for more information. Thanks for your comment!

Our skills book provides step by step instructions so that you can create your own evaluator when you are practicing at home and know if you are doing the skills correctly. +margarita Lewis Since you are done with the skill, your gloves are soiled. Dirty gloves can touch the faucet (faucets are always considered dirty) - you do not need a paper towel to turn the water on or off. You can use one of you want, but it's not necessary. Hope this helps!

You must wash your hands AFTER greeting the patient and getting permission but BEFORE gathering your supplies. I'm not sure what you mean by preparing the toothbrush first...before what? You must prepare the toothbrush before brushing teeth. But you cannot prepare it before doing your opening and washing your hands (it's presumptuous and you don't have clean hands). Hope this helps!

Yes. Think of it this way....if you were the patient and someone had to brush your teeth, do you want them to wear clean gloves or gloves that have touched a bunch of things like toothpaste containers and toothbrush wrappers? By doing all of your prep work first, and putting your gloves on immediately before brushing the teeth, you are keeping your gloves clean for the patient. Hope this helps!

There are two different bed adjustments - the entire bed and the head of the bed. At the end of the skill, the entire bed must be in the low position, but the head of the bed can be adjusted to patient comfort. I say yes it follows the exact steps as the California one. I am testing on Saturday here in California and I have passed before but testing again because it expired and I did not work as a CNA and if not do the educational credits. I remember having mouth care as one of my skills when I tested and I did it this way. I am reviewing anyways.

These videos are set up exactly like the exam. They are based on the exam scenarios. So, if the person is in a bed at the beginning of the video, that is where they will be for the exam. If they are lying flat, that is how the exam will start. These videos were created to give you a realistic representation of the testing procedure. Hope this helps!

AFTER the skill has been completed, your gloves are soiled. Soiled gloves (and hands) can touch the faucet. You only need a barrier between your hands and the faucet if your hands are CLEAN. Hope this helps!

I’m waiting to find out if I passed or failed because I got this skill and followed it but I forgot to wet the toothbrush and I probably failed 😫

Yes, for the exam, the "patient" won't be there tomorrow, so the toothbrush will be thrown away. In a clinical setting, you will not throw the toothbrush away after each use - you will simply rinse and store it as you do your own. Hope this helps! :-)
