Anti-double stranded DNA antibodies (anti-dsDNA) are autoantibodies against extracellular double-stranded DNA...They are associated with systemic lupus erythematosus (SLE)...They are very specific for lupus, yet, they are not very sensitive for SLE.
Anti-cyclic citrullinated peptide (anti-CCP) are autoantibodies that are present in the serum of most patients of rheumatoid arthritis...These anti-CCP antibodies are not sensitive, but they are very specific.
Citrullination is a method of post-translational modification (PTM) in molecular biology...Modifying arginine into citrulline via deimination (not to be confused with deamination).
Rheumatoid factor (RF) is an IgM autoantibody against the Fc portion of IgG. "an antibody against an antibody".
Rheumatoid factor (RF) is present in 80% of patients with rheumatoid arthritis (RA), which makes this test 80% sensitive.
Rheumatoid factor (RF) is not specific, because it can be seropositive in many diseases including rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), sjögren syndrome, interstitial pulmonary fibrosis, hepatitis B infection, essential mixed cryoglobulinemia, infective endocarditis (subacute bacterial endocarditis or SABE), tuberculosis, infectious mononucleosis (mono) and hematological malignancies...This, by definition, is non-specific.
Anti-neutrophilic cytoplasmic antibodies (ANCA) are IgG autoantibodies against the cytoplasm of the neutrophils and monocytes.
This is different from antinuclear antibodies (ANA) which are antibodies against the nucleus.
Anti-neutrophilic cytoplasmic antibodies (ANCA) have several patterns; the most important two being the cytoplasmic ANCA (c-ANCA) and the perinuclear ANCA (p-ANCA).
Anti-neutrophilic cytoplasmic antibodies (ANCA) are associated with vasculitis, such as granulomatosis with polyangiitis (Wegener granulomatosis), microscopic polyangiitis, eosinophilic granulomatosis with polyangiitis (Churg-Strauss), rapidly-progressive glomerulonephritis (RPGN), anti-glomerular basement membrane antibodies (ant-GBM or Goodpasture's syndrome), as well as inflammatory bowel disease-associated. vasculitis.

Is it because that, if TNFà is inhibited , anti-dsDNA will be increasing? Then, give the patient balilumab to knock down the nasty antibody!!!! Right?

It is when patients with RA taking TNF-alpha inhibitors because intake of TNF-alpha inhibitors may show as positive Anti-dsDNA. Just started watching medicosis perfevtionalis and it really helps me study. thank you so much

6:20 you mean less specific? Since normal individuals can have a + dsDNA test (false positive result) that would lower the specificity, not the sensitivity (which suffers under false negatives).

How can it be a very specific test if normal individuals can have positive anti dsDNA?

So let’s say a patient has a low pos ANA, but normal CBC labs and such. If a patient has a pos anti-dsDNA ELISA, but then a negative anti-dsDNA CLIFT, would it be considered a positive if they seem to have symptoms? I get the difference between the test types (one is more sensitive and one is more specific), but not sure what one pos over the other would suggest…

How can someone have negative ana and antidsdna if it’s part of ana? I’m the rare one sadly. I have cutaneous lupus and consistently positive antidsdna so they diagnosed me with lupus-like disease. I also have a malar rash. They are now checking me for neurolupus because my symptoms are rare and atypical.

thank you for your great Videos they HELP so much, and for the answer: Because That patient who has RA may be on treatment with TNF-alpha like infliximab so if a patient is recieving TNF-alpha for any reason they may show positive Anti-dsDNA?

Antibodies are proteins (i.e. water soluble). Water soluble molecules are still capable of diffusing through the membrane (especially if they are tiny particles), but very slowly. Another possibility (this one, I am not 100% sure about), if a cell has antibodies on its interior, then during cell division, it’s possible that the new cell will borrow some of the antibodies.

@@MedicosisPerfectionalis I read this afternoon some articles about my question, and actually it's complex. Even in 2020 the mechanisms are not completely understood. I have found a new concept called the TransmAbs ( monoclonal antibodies ) which is very interesting, and other things.

In Robbins Basic Pathology they say that there is no evidence for Anti nuclear antibodies(ANAs) which are involved in immune complex formation entering intact cells. What happens is defective apoptosis leads to exposure of nuclear materials where then ANAs can bind.

I just scored equivocal (5) on the Anti-dsDNA test. Does anyone know what this means? Additional info: Negative ANA and complement C3 serum is 101. My kidneys "look fine." Have high LDL which is bizarre for a young, active, whole-foods Mediterranean-style diet person.
