As one who has resuscitated hundreds of pts. over 30 yrs+, I would not be quite so dogmatic. Asystole occasionally does respond to a chest thump or 2 and to low energy (5-10 watt-sec) non-sync. shocks. It's seen in the OR post cardiotomy almost universally. Pts. who are acidotic will respond very poorly to epinephrine. Restoration of productive rhythm will be much less successful in the acidotic pt. Gentle compression of the splanchnic bed can be very helpful in increasing central volume esp. in obese pts. thus increasing the effectiveness of chest compression. VT should not be shocked with high energy at least not initially. Except in obese pts., 10 watt-sec. or so should be tried first. Asystole can sometimes be paced successfully. DON'T give up easily. I once resuscitated a young female who had already been pronounced dead by the "ER team". It took 6 hr. but the pt. made a complete recovery. That was the record for that particular hospital. I also set the record of 7 hr. at a 1000 bed medical center. That pt. also survived without any ill effects (surprisingly) and lived for many years. I could go on...and on with additional recommendations and approaches but that's a start.

My brother-in- law just died with this. He went into cardiac arrest and I did wonder why they didn’t ‘defibrillate’ him. I guess then that asystole had happened and had no heartbeat whatsoever. Sadly, paramedics were unable to revive him and he passed away. This has answered some questions for me and it has helped me to understand what happened. Thank you.

Kindly clear my doubt sir. If cardiac arrest happening outside hospital, we can give cpr and once ambu bag arrived we give ventilation and on arriving hosp we do the rest.. But within the hospital, pt had arrest and all facilities available. So how minutes can i perform initial cpr and when do i perform ecg? How frequent to check pulses and serial ecg? Help me with this sir.

A lot of effort has been put into making this video and I appreciate that. However, the deliverable is almost zero. I say this because of the following reasons: A person may have these problems either at home or in a hospital. If the person is already in the hospital then the medical staff will take care of it. However, if the person is at home (which is more likely), then I do not have the ability to revive the person using the instructions on the chart. It has to be noted that I have merely 2 to 3 minutes to act. Hence calling a doctor would not be my highest priority (unless there are others whom I could instruct with zero time loss). I would merely try to revive the person by chest compression and making the person breathe. My suggestion is that instead of making this long video, just tell me the minimal steps that I have to take to revive a person in most cases. I know that my diagnosis may be wrong a few times but that is the best that I can do.

Gasping for breath can occur in cardiac arrest (in at least 40% of witnessed out-of-hospital arrests). Therefore, the presence of breathing attempts or effort does not rule out cardiac arrest.
