Even if he doesn't do it for enjoyment, or he doesn't die, drinking bleach to get sick either sounds like he's trying to get some enjoyment by resorting to becoming a masochist, or he's depressed because of all the bleach constantly makes him sick.

Going by TF2's official lore, Demoman was an orphan, then accidentally blew up his adoptive parents, then got new parents who forced him to work multiple jobs and intentionally avoided emotional attachment, and during one of these jobs he got irreversibly disfigured by a wizard. His best friends are a lead-poisoned lunatic and a possessed sword, and despite owning a mansion he STILL lives with his emotionally abusive mother. The man has good reason to be depressed.
