Thats exactly how I see this video is portraying, they had a big fight, but before they could resolve it, he died in that horrific car crash, that is what is killing her as well as the other lyrics the song is actually saying.

My theory is a bit different than yours. So, judging from the video and the song and which lyrics are played in each scene I think the car might be the relationship. So hear me out right? The beginning of the video has her walking out of the car yes? She leaves him (in the relationship) and he carries on being destructive, driving too fast, not paying attention to the road. He runs into her (the truck) and dies. His ways got him killed because she found someone else or she finally stood up for herself and her needs in the relationship. He couldn't give her that and the relationship died. The lyrics about him cheating and her trying to decide if she should start something or be a lady out of love for him appear right when he's calling someone (maybe a side) this would also point out him destroying the relationship. But idk, it's a half baked theory, do with it as you will. (Great video by the way).
