I didn't quite get it. Should the dentist be responsible or another staff member for spreading referral cards and asking patients if they have a family member that hasn't seen a dentist in a while

Ideally the OM or someone in charge of PR for the practice would be the one running this. With a smaller office - someone - doesn't matter who should be responsible. The key is one person is in charge of making sure it happens and ensuring that team members are doing it.
