That thing has LORE, summary? Steve got infected with a parasite while in a cave that did these mutations and made him kill everything that he loves (could be friends, family, pets) and he has no control over it. He also says "It's stretching my skin to the limit", so he most likely is also in constant agony from basically being stretched to the point he's almost been torn apart
