In my au what happened to Mrs. Afton was: Mrs. Afton cheated on William a lot of times so he couldn't take having an unfaithful wife. He didn't have the money to divorce her, and felt like she needed something more extreme. He thought if she was dead, he wouldn't have to split money with her. he didn't know how she would just drop dead, so he yelled at her and killed her with the knife he used to kill the missing children. this put the souls in one place, eventually Mrs. Afton's soul went into Mangle along with William's bullies at school who thought Michael was teenager William and wanted their revenge.
