I was born with missing upper lateral incisors. I was advised I couldn't have dental implants as there was not enough bone width . The dentist told me I would need braces to push the gap further back, then place dental implants which he said would be very expensive. Consequently, he recommend two fixed dental bridges which ground down my four natural teeth. I regret this greatly and wish I had just left the teeth and gap as they were

I'm missing one, so my canine moved forward in its place. I got lucky re bone retention though, still have my canine milk tooth somehow lol. We'll be masking the permanent canine to look like the other incisor and then removing the milk tooth and replacing it with a canine implant after the space has been opened up a bit more with braces.

Sometimes there aren't that many options and the position of the teeth leads to one clear best option. Instead of maryland bridges, spaces can be filled with dentures, conventional bridges or implants.

Answering this question needs an assessment with an Orthodontist as it depends on the configuration of your bite, your facial type, your jaw structure, size and colour of the canines, the gums lines, whether the laterals are actually missing or just impacted and never spotted etc. so I suggest an orthodontic opinion. You need an X-ray to know if the tooth is hiding there or just never formed. Lateral incisors are much more commonly missing than stuck. Hope that helps!

Can canine substitution cause Temporomandibular disorder? I felt my jaw fall slightly forwards and this has now been followed constant jaw clicking above my ears for days , jaw fatigue, mild pain and sharp pains when I chew sometimes. The only way I get relief is by pushing teeth apart with my fingers for a few seconds to oppose the force of my braces (similarly to how a dental palate expander works). I am going to contact my orthodontist but I was wondering if they would be able to reverse the canine substitution and do implants instead to avoid anymore possible jaw complications and expand my palate. Would still be covered under the NHS as I'm 19 now. I know you cant diagnose or give medical advice over the internet but any opinions would be appreciated. Thanks.

Dear Coaly boi: There is no evidential link between orthodontics and TMD, please see www.cochranelibrary.com/es/cdsr/doi/10.1002/14651858.CD006541.pub2/full/es The systematic review does however highlight the need for more evidence. I have had patients mention that their lower jaw feels trapped inside the upper before treatment but this is only anecdotal. I have never had a patient complain of this following canine substitution. You're right however it's impossible for me to give specifics on your case so well worth a chat with your Orthodontist who I'm sure will do their best to help! They are experts at this. Good luck!

I'm currently getting the canine substitution and have just received elastics yesterday to maintain my bite. I was wondering if the elastics change the aesthetic of the jaw and how noticeable is it?

My only concern with elastics is that they could cause by lower jaw and chin to recede as they move my lower jaw and teeth backwards and therefore ruin my face/jaw aesthetic. Two months ago I decided to put on fast-braces and now i have to come to a decition on whether to close the gaps with braces or leave a gap for an implant or a mairyland bridge. I was considering the mairyland bridge but after research I found out that my bone and gum might recede if there is no support between my front incisor and my canine. Because this is gonna be in the front of my mouth, i dont want my gums to recede and look bad. Also i dont want to put metal in my bone and have an implant. Should I just close the gaps and unfortunately end up with a smaller mouth? Or is the mairyland bridge a good solution?

I do have some missing teeth in my upper left first molar, right central incisor, and right incisor. also in my lower right and left first molar. I really wanted to put braces on it because the implant is expensive. I do regret removing my crooked teeth, which are my right central incisor and right incisor. When I also asked my doctor, he said that I could get a dental bridge to close the gap, but I don't like it, especially in front of my teeth, because when I smile, my gums are visible. But I'm fine with using my other teeth to be the foundation so that it will not be obvious when I smile. I am hoping I can get a reply so that I will know if there is still a possibility that I can close the gap or substitute my other teeth to become the foundation so that I will no longer do the implant. I been thinking for months about this and its stressing me out.

Dear Yuki, large gaps are often possible to close with braces but need to be assessed by an Orthodontist in person to assess the feasibility. I suggest booking in with a Specialist Orthodontist local to you. Good luck!

Sorry for the late response I missed this comment! Teeth move roughly 1mm a month so that can give you a general idea but it's hard to give more details without seeing your teeth. A question for your Orthodontist. Best of luck with it all!
