Eh, not really TBH, I got super addicted to tons of stuff in my survival playthrough, namely jet lol, it isn't actually that hard, it's just debuffs. Even if you have 1 in every stat, it's just a patience game, not difficult. You still have full access to weapons and armor, so it's 100% doable, and probably doesn't even meet his desire for self inflicted misery lol.

Beating survival mode without eating, drinking and sleeping, but with some of the toughest addictions could also be interesting enough. It worked in Vegas, it might work in 4 as well. I'd be curious if the game even gives you enough time.

Here's a list of random ideas for future challenges: Beating the game blindfolded (as you mentioned), using only one hand, speed running the game as fast as possible (without looking up a bunch of strats) just to see how you compare to people like tomatoanus, a run in which you kill every NPC you can find, using only the keyboard OR mouse not both, a no item run (weapons, armor, health items) could probably be combined with another challenge to make it a bit more interesting.
