The article "the" only could replace the possessive pronoun "his" in this sentence. (It was the most terrible mistake). But the gap asks for something different. It asks for an adverb. And this adverb is "none".

@@ud8606 I've just looked it up. "None" in this context means "not at all". Here is what I found on the net: none. adverb. /nʌn/ /nʌn/ used with the and a comparative to mean 'not at all' So this means: It was not at all his most terrible mistake.

@@ud8606 I asked an English native speaker, a friend of mine, about this weird "none most..."-expression. She said this may perhaps be an 'old English'-expression but definitely none in common usage! So I think we better forget about it!
