It has happened to me. Sometimes you just forget to click a ball/strike, like on a passed ball. Foul ball followed by a coach calling for your attention. A pitch that you call a strike, while a steal is being attempted and then something weird happens on the play that causes a stoppage of play. And you had not clicked the strike on your indicator. And here you are, late in your second or third game of the day and have seen like 1000 piches(Not exaggerating). It can be difficult to recall. And usually your base partners dont carry indicators. As an umpire you have so much game info in your head your brain just sort of dumps redundant information. It happens on a subconscious level I think.
