Not sure how hear of fire would help (didnt see how it would help when it came up on my replication of the challenge). Doing it on the carlo cart level to force more enemy interractions. Went pretty well even with maxed out curse and charm. Got the healing arcana and the sanctuary one as well.

The explosion that Heart of Fire creates when enemies damage you counts as retaliatory damage, so you generate a significantly higher amount of empty health when combined with Divine Bloodline than you do without it. Other than that though, HoF is pretty weak overall.

Can you do this without all the powerups and stuff that you added?

Unlikely. The Powerups are very integral to being capable of doing remotely anything challenging in this game.
