They say modal verbs are always followed with a base form of the verb? But lets say: His Party will be planned carefully. any explanation?

"Will be planned" is in passive voice. That is--- "be" verb + past participle form of the verb. Whereas, the active voice is "will plan" Ex: His friends WILL PLAN the party.

maam pag binigyan po ako ng topicng teacher pero sabi nya lang po use modals ano poyung modals nagagamitin verb poba auxiliary,adverb, noun lahat poba yan

Most probably, verbs yan. Pero much better to ask your teacher to know exactly what he/she expects to see since there are other types of modals (noun and adverb) based on this video.

wasn’t is also there, by the way. i think the reason why “couldn’t” was there was for the sentence “i couldn’t walk until i was 3 years old. ”

Oo Mali yan but in using modal terms ang pagkakaaala it contains total of 8 errors that you can found in pronunciations, spelling and grammar.

@CV We must not use CAN/COULD and ABLE together because they both mean "ability". You can use either of this 2: (1) I could not + verb (2) I was not able to + verb
