I have never met a CFII that teaches complete ICAO-style use of an IFR flightplan, and never had [FAA-]ATC reject an ICAO form that only contains the basic info that is found on an old-style domestic IFR plan. (I'm not saying this is a fully acceptable teaching practice, but that is a common practice even at 141 schools.) And anyway the FAA's adopted use of the ICAO form is not 100% ICAO-compatible processing anyway. Not that the ICAO version is all that great either, as the format is a bit of a haphazard mess of arbitrary fields and vague redundancies. It would be nice if the FAA released a comprehensive guide to formally fill it out, with complete definitions and explanations of examples (the quick reference cheat sheet doesn't cut it). For example, ICAO wants any planned delays at a fix (eg holds or just a block for practicing instrument maneuvers) in field 18. The FAA system will ignore or totally discard the "DLE/xxx..." syntax in field 18, and wants delays included within the field 15 route list using a different syntax.

You have a good handle of things. I agree with you on all of your points. I will say that ForeFlight has the best guide I've come across for filling out the ICAO form.

Very nice! In the field 18( other info) the FAA sheet and foreflight does not let select Z at the field 10a if you don’t have NAV, COM or DAT at field 18, but I don’t know what options would be there. Any idea?

You're correct, Z is only displayed when you have appropriate equipment selected in section 10
