Hey Michelle! I’m about to graduate from a doctoral program in nurse anesthesia. Another thing to mention with people wanting to have weight loss surgery while morbidly obese is that anesthesia can be more risky. Ventilating, intubating, and sedating morbidly obese people can harm them if you’re not careful, because the excess body weight decreases their lung reserves and requires a lot more pressure, and the extra tissue in the throat contributes to them obstructing their airways when sedated. I wish more people knew this when seeking out surgery!
@meghanodonoghue9066 Жыл бұрын
That is fascinating! It makes complete sense, but is probably overlooked by fast trackers.
@tedtom1566 Жыл бұрын
That makes sense. So does that mean anesthesia is pretty safe for an average sized person? What about a severely underweight person? Just curious.
@birblover6423 Жыл бұрын
Congratulations on your future degree!
@SavannahSamuel Жыл бұрын
@@tedtom1566 always come with it’s risks! But it’s easier and safer. Underweight people are going to have malnutrition and protein deficiencies which can mess with our drugs. But their airways are easier to secure for the most part. When we put someone completely asleep for a big procedure (like gastric bypass) we sedate them to the point where they lose their airway reflexes, and can paralyze them in order to intubate and take over their breathing for the procedure This leads for potential minutes where someone is not breathing while we do this. For an average person who is preoxygenated well, you have about 8 minutes of “apnea” time before bad things start to happen. Due to decreased what we call “functional residual capacity” obese people (as well as with neonates and children) have way less time, and desaturate quickly. So you’ve got a combined potentially difficult airway with the excess tissue, difficult ventilating, and little oxygen reserve for time to do it for morbidly obese people. Bad things happen faster.

My mom at one point claimed she had quit drinking alcohol, but when I asked her how, because I was struggling to quit without going to AA myself, she clarified and said, "oh well, I quit drinking liquor, wine and beer don't really count, do they?" Another family member said she stopped eating "anything white", so no "white potatoes, white rice, or white bread", bit still ate cake once a week and donuts once a day! I think these people need a year or establishing and ramping up to what he wants. I know these are emergency situations and time is tough, but some people just cannot do it on a shorter time scale. There’s a level of delusion that’s just untouchable by his techniques. It’s basically substance abuse with these people.

@@andianderson3017 yes, addiction is an issue and not wanting to take responsibility for their actions also. When Dr. Now asked her "what's surgery going to do for you?", she either didn't realize or admit that SHE would need to change what she eats and the surgery would FORCE it for a while. When people don't realize and are able to get the surgery, the newness of the surgery will make them drop fat, BUT if they don't change and continue eating more than they should on a constant basis, their organs will increase in size (specifically the stomach) again.

@@larasommer5707 the definition of malnourished can also refer to overconsumption of any kind of nutrients and calories and I'm not sure if that's what you meant because it is generally true that pretty much all overweight and obese people can be considered malnourished but it's because they consume way too many calories and probably macronutrients. It's very hard for the average person to get all of the nutrients they're recommended to consume so most people are slightly malnourished in some way anyways. Came here to say exactly that. My grandmothers friend deliberately gained 100 lbs to qualify for gastric bypass, and then would not follow the diet after her surgery and ended up eating an entire watermelon and ruptured her insides and killed herself. I got my ass in gear, followed the next-to-no-carb diet they gave me, drank water and crystal lite ONLY and in one week lost 14 lbs. Granted that was almost guaranteed to be water weight but it gave me a little sense of accomplishment when the nutritionist I was meeting that day did a double take at me and asked if I had lost 14 lbs in 7 days LOL But yeah, I've gotten down to 286 since surgery, and at surgery I weighed 330 lbs, so since my highest that they measured me (not my highest weight, which was 420), I've lost 103 lbs. And it's been the hardest shit I've ever had to do. I throw up so often, my nausea is crazy and my mentality around food has changed entirely. For the better, I'd say if I'm honest. I don't have nearly as many cravings as I used to, but I definitely still rely on food emotionally, so I'm needing to look into a therapist for that aspect of it.

Worked as a personal trainer in the past and it really put into perspective how tough it is for the average person to lose weight. For the first few months I did the typical thing, “Oh, everyone had a choice, the law of thermodynamics, calories in/ calories out, etc.” after a few months I realized not everyone was a health and fitness fanatic like my peers and I. Losing weight means changing your relationship with food and that’s the hardest part for people to grasp.
@matthewcooke4011 Жыл бұрын
I completely agree with this. It can be very DIFFICULT for people to lose weight because, as you say, it's not as easy as just vowing to eat less. Many people are addicted to their eating habits. However, the calories in/out principle is still true. When people say that they CANNOT lose weight, no matter what they eat (or don't eat), they're simply lying or self-deluded. My ex-wife was often morbidly obese, but she never claimed that it was not her fault or responsibility.
@lilalizzard Жыл бұрын
I‘m glad you got that perspective. It can even be dangerous for a seriously unfit person to start working out bc they really start from zero. Practices that are easy warm ups for „normal“ people can lead to pulling your muscle and that putting you out of exercise for days, even weeks for unfit people. I did a „beginner‘s“ work out on youtube to get moving, I pulled my thigh muscle and couldn’t walk for days and couldn’t exercise my thighs for nearly three weeks. That shit is depressing. When you are on that extremely unfit level like I am, you need to start slowly and be very careful. I stopped trusting anything labeled for beginners on youtube bc often times, it‘s not. The creators don‘t realize that they‘re already super fit and their easy warm up will probably not be easy for beginners. But if you don’t know shit about exercise like me and people tell you to „just get moving“, injuries like mine are prone to happen as it‘s not as easy as „just moving“ makes it look. Found that out the hard way, really.

People don't know how to deal with the feeling of hunger. People eat when they're hungry and eat until they're full. But they often don't consider that having an appetite does not mean you need food. They think about eating in terms of how they feel, but really it's a math equation. Finding ways to make that equation pleasant can be hard, especially considering how most people don't know how to cook their own meals. People pour sauces and use tons of oil/butter on everything and don't know how to develop flavor with seasoning.

I know three people who had weight loss surgery- every single one of them failed within 6 months. They just stretched their stomachs back out. One of them was left with lifelong complications meaning they can't take certain medications such as pain killers (even paracetamol and ibuprofen), the other still vomits whenever they eat high fat food and its been 4 years. This isn't an easy, magic operation that will solve all problems and everyone skips off into the distance. It's a serious procedure with lifelong repercussions regardless of whether its a success or a failure.

The thing she said about replacing one addiction with another is so true. I moved out in my mid teens and ended up experimenting with more things more than I should. When I moved back in with my family at 19 I wanted to be a better role model for my younger brother, so I cut all that out, but my smoking went from the occasional party cig to secretly chain smoking in the forest behind the house. I was previously diagnosed to be pre-diabetic for Type 2 but I lost weight with exercise, changed my diet long term, and was moved out of that range. Since becoming pregnant, I lost another 17 pounds in 3 months, despite the fact that I am expected to be gaining weight. I was also diagnosed with gestational diabetes which is being very well controlled with minimal medication and diet tweaks. Less carbs, more protein. But our baby is developing fine, I am just trimming down while eating healthier and getting gentle cardio. The morning sickness also helps appetite control, gotta admit... There’s a mental health evaluation, you have to meet with a dietitian, multiple tests and blood work needed, and meeting with the surgeon who will be doing the surgery. It really takes a village to help with your weight loss goals. My family was such a big help because when I wanted to eat unhealthy they would encourage me to stay on the path. At family barbecues my family would have healthy options for me so I could stay on said path. I started at 420 I lost 40lbs prior to approval, and another 20 before surgery. Hi Michelle! I love your vids and I wanted to add something that nobody says but it’s important!! I am a med student (almost done) and if somebody gets a gastric sleeve surgery and after that they loose control and eat too much the stitches can tear apart and the stomach gets perforated. This is potentially lethal ‘cause the gastric acid can literally dissolve the other organs (you basically digest yourself). Furthermore, performing surgery on an obese body is extremely difficult (the organs are all displaced by the fat, the operating space is not at the best “distance” , not everything is at arm’s lenght, the organs are already suffering and there is a much higher chance of respiratory failure with anaesthesia AND the body’s overall ability to recover is compromised due to the chronic state of inflammation related to obesity). All in all, weight loss surgery’s not for everyone, you have to prove you can control your eating not because “you would make the surgery futile” but because eating too much could kill you (instantly, this time). Obviously my comment is not meant to say that weight loss surgery is a negative thing, it saves lives! I only wanted to add a few elements that nobody ever mentions…

I barely eat, but I've been in a really low place since last summer. I started working out a lot around September, and I've gotten in the best shape of my life, but I've still had such a negative attitude, and I've had crying breakdowns about every single day. I really missed the validation that I got from my ex girlfriend, and I couldn't accept the fact that she had left me. The fact that she did it two weeks after my dad died didn't help. After I lost my job the next week, because my performance had declined since my dad died, I really spiraled. I tried drinking at first, but thankfully didn't relapse on hard drugs. I've been clean for over four years now. I stopped drinking because I didn't want the calories. I couldn't choke down vodka without a mixer like Mountain Dew Spark. So then I really got into working out instead of drinking. That was probably a good decision on my part. I just decided the other day to face reality, to have some self respect, and stop being such a whiney shit. I haven't checked my phone notifications or text messages or facebook notifications or anything since August, since my ex's new boyfriend texted me to dump me for her. I let it break me. I really fell apart, and that's my own fault. One thought that has really haunted me since last summer is that I will die alone without ever finding someone I thought I loved again. I realized that I need to be comfortable enough in my own skin that I'm fine alone, and stop being such a catastrophizer. Sorry for the long comment. It feels cathartic just to get this off of my chest.

Hey, you experienced a LOT of trauma in a very short space of time and it’s okay to grieve your losses, it’s okay to feel sad and like your world as you knew it has gone and will look entirely different from now on. Try to be kind to and patient with yourself, healing takes time and that’s a cliche because it’s true. 2002 for me was the worst year of my life because like you, I experienced a lot of trauma within a short space of time and I also broke down. I also made some bad decisions because I was so emotionally torn apart that I couldn’t think straight. I’ve just turned 52 and am living alone for the first time in my life and I can’t see a romantic relationship in my future and I’m okay with that. My last one was abusive and ended 8 years ago and left many emotional scars that will always invade my brain regardless of how much therapy I’ve had or could have. I’m not hanging on to those thoughts because I want to but there are small things like a song, place, smell, even just a simple word that will never stop being reminders and unless I have a lobotomy that won’t change. It’s okay to complain about how you’ve been treated and what you went through and you’ll only get better when you’re ready, you can hasten the process a bit by doing all manner of self help things if it works for you but there’s no time limit on how long that’ll take. I wish you well, remember you’re not alone 🤗

We had a show in the UK where they took someone's self-reported calorie intake, and and then installed cameras in their workplaces, homes and vehicles to monitor what they actually *did* eat. A lot of the time people weren't being deliberately dishonest, they just didn't think it "counted" as it was just one or two little things. But those little things add up.
Some people cannot be helped. Their denial, lies, laziness is who they are.

You gave me an epiphany today. Honestly, it’s something I never considered very much (sometimes it’s more obvious in others), but I’m willing to bet that the majority of people with over-eating disorders are doing it to mask depression- because it’s easier to eat than admit your sick. You said she, “Praises mediocre work,” and I went, “OOF…..” I was kind of RAISED to just put in the minimum efforts to survive cause I have severe depression and BPD, so when people eventually started to give up on me getting better, people started just telling me, “Do what you can… It doesn’t matter if it’s only a little bit.” I’m not saying that it’s a better excuse than anything else…. But from my perspective, if I got to that size….. sadly, I probably would get stuck at that size too. It’s hard for me to have motivation now as it is- but it’s not me that’s too heavy, it’s my brain… 😭

I have autism and ADHD, so it's difficult for me to function normally or do ridiculously simple things most people don't even think about. And when I was diagnosed it was a relief for me, because it was like my "get out of jail" card. It was permission to do things my way and to step back if it was too much for me. And that's good, I needed that. Still, I had to understand that my life wasn't going to make itself without me making any efforts. The people in my life may be willing to adapt to me and my rythme because I am autistic, but life wont. If I want a diploma, if I want a job, if I want to be fit, if I want kids or a relationship I will still have to work for it, same as everyone else, same as before. It's about finding the right balance, you have to cut all unnecessary stress from your life, and give yourself the right to set your boundaries. But you should work hard for the things that are important.

Sometimes when I’m in a depressed or anxious mood, I end up overeating/eating too much junk food (though not to the extreme many of these patients do) and other times I don’t want to eat. I've often wondered why counseling isn't paired with weight loss programs. It seems a lot of weight issues stem from psychological issues. I feel like mental health is severely undermined and the threat it poses to our lives and the lives of others needs to be addressed. Behaviors in excess can stem from a variety of different mental health conditions that like hoarding isn't likely to equate to a healthy, lasting change without recognizing what starts the pattern, what helps break the pattern, and not spiraling into the hopelessness that continues the pattern.

But Dr Now probably has at least one nurse who's trained in taking blood from rolling veins -- most clinics do -- so if she'd said that, he could've assigned her to a nurse who knows how to do it.

I stopped working out for 2 years during COVID (I exercised regularly for 25 years- I had a six pack in 40s!), and gained 10kg (I'm 53, male). But I was honest about why- sedentary lifestyle, eating too much (especially comforting junk). Now, I'm back to cycling and weight training, and eating healthy again. It takes some effort to make it habit again, but the health benefits are so worth it! The funny thing is, when I mention I'm a bit overweight, my family and friends act like I'm just beating myself up, and I'm just fine the way I am, blah blah blah. I'm NOT beating myself up, or feeling depressed about it- just stating a fact. I'm overweight and need to work on my health (which I'm doing). Excuses and enablers are the worst!

I want a segment where they lay out a bunch of meals of different sizes and see if they can pick out which one is an appropriate amount of food. Tbh, when I lost weight and finally got down to my goal weight and took a look at how much food that was...I was floored at how little it was. Going from the mindset of chronically overeating as being "normal" and then learning what a normal amount of food axtually is is really jarring.

I’ve been here before and I was eating too much, 1500 calories a day especially if your eating calorie dense foods is really not a lot.

So true. 2k calories is not much if a person is eating even twice that let alone 10 times. Portion size is a constant challenge.
@Lacroix999 Жыл бұрын
It’s healthier to eat until you’re about 80% full and stop. It’s not a lot of food because your stomach isn’t that big. The thing is we eat when we’re not hungry for emotional, social and boredom reasons, not to necessarily sustain us health wise. The can ‘train’ our stomachs to stretch or shrink when needed. Farmers do this with their cows when they know their livestock will be going through a famine. They slowly decrease their food given so their livestock doesn’t starve instead of cutting them off abruptly which can starve them. If you stuff yourself constantly, your stomach will stretch to accommodate that amount of food (to a certain degree) and if you do the opposite and only eat smaller portions, your stomach will get used to doing the same. Have you ever stuffed yourself more than you should have and more than usual one day before bed and woken up the next day ‘starving’? This is why. It’s because your stomach got stretched out a bit more than usual. Same for not eating enough for a while. When you eat more after that, your stomach gets fuller faster. These people are accustomed to eating more all the time. To give the smaller meals, makes them feel like they’re starving themselves and not getting enough to eat, making them want to eat even more. On top of this, they are wanting to eat more because it’s the one thing they can control in their lives because they have other unaddressed and unacknowledged problems in their lives that also need to be deal with when goi g through this process. Cutting off the food is only half the problem. Working through their mental issues as to why all that exists is another.

It's hard to measure less than 5lbs because we fluctuate and have food in our system most of the time. You can go up and down 5lbs without actually losing or gaining any actual mass. Fasting even if just from 6 at night till 7 or 8 in the morning will help in many ways. Slow weight loss should be around half a pound every 2 weeks, give or take, which would be 12 lbs a year. I’ve always had those intense cravings. I always described it as “the two different hungers”. One that comes from my belly and one that just screams at me until i eat. People looked at me weird when I tried to explain it to them. Turns out I have Adhd. One hunger is actual hunger , the other is my adhd brain trying to compensate for dopamin. I learnt to meal prep every meal, count calories. and excercise to just maintain a normal weight. I am not overweight. But I am also not skinny. It always bothered me that I had to be so meticulous and i just didn’t know why. I mean, yes, i had to consume about 2000- 2200kal to maintain some sort of chemical balance and I knew it was higher than what I should eat. But every time I went lower, my cravings went out of whack. I honestly could not get how people just eat 1200 cal. Now that I am diagnosed and treated, my hunger makes so much sense and is so much easier to manage. I am not saying that as an excuse but an explanation of why some people struggle a bit more than others. Obviously this lady is delusional and takes no responsibility for anything.

An extremely protein-dense breakfast is a great way to keep dopamine in check for the rest of the day! I'm talking 20-40g of protein in the morning. But lately I've been having 30-40g as a midnight snack before bed instead of breakfast because I've been working out a lot, I sleep better and I'm not as sore as I expect to be after weight training day. As I continue through this journey of battling B.E.D., I'm realizing that the part that is often mishandled or neglected is the addiction aspect of it. Personally, it feels very emotionally similar to drug addiction in the sense that when I am experiencing high levels of stress or turmoil, I am triggered to binge until I "feel better". I'm getting a hell of a lot better at managing both my mental and physical health, and a long with it my urge to binge has continued to decline. With that being said, I'm certainly at a stage where I do "fall off the wagon", but I at least employ harm reduction strategies such as working out, making sure I'm hydrated, and making sure I throw out any leftovers immediately so that I can't consume the unhealthy foods. I remind myself that while I can't out work a bad diet, I can hold myself accountable and keep up my physical routine instead of throwing the baby out with the bath water by giving up.

@@w1975b Thank for the suggestion, but I do prefer a more balanced approach. I do generally enjoy calorie counting because overall, it is helping me reevaluate my relationship with food as a whole. The other reason that it is not really for me is because I am a "savory" gal as opposed to a "sweet" gal, and things like red meats, starchy vegetables, decadent cheeses, etc., are the types of foods that are my "drug" of choice. If you put a whole giant ass 40oz rare tomahawk steak in front of me, I'm going to do everything in my power to stuff as much of that into my body as possible to an extreme excess... which you know, is definitely not good at all.

@@darlingdame_x187x I mentioned B.E.D. being helped through carnivore and giving you a source for seeing the proof. Another is Carnivorous Me's channel. Bart Kay did a priming challenge for a month iirc, and he STUFFED himself with only meat, he was so tired of eating. Of course, he was coming from omad, but still, he is a nutritionist and states very clearly that it is VERY DIFFICULT to overeat when all you're eating is meat and the fat that comes with it. He does consults also. "balanced" is a feel good term, there's no scientific definition in terms of diet/nutrition.
