The way you mainly use c4 like that is when Snake is in mid-air he is able to place a bomb on an enemy. (Cannot be blocked) Another thing if you not paying close attention you can also stick it on an opponent just before they hit you and it near impossible for the opponent knowing it is own them untill you detonate it.

I feel like the best thing for a snake main is very quick planning, reading, and punishing. With all that they did in this vid was completely coordinated, read. If you grab onto the cliff and climb up/roll on the ground, you get whacked by the grenade, if you jump up off of ledge, he is waiting for you there. It's pretty hard to beat a snake when they have a third eye
