Most likely case is either the checkpoint theory of the crazy man who pulled this off or a game state change error. For example in the 1993 doom game, when you end a level, the game state changes to the level transition state, and when you load in to a new level you are in the play state. If ultrakill works similarly, when you enter the shaft the game has you in a state that does not expect gameplay, and then dying and respawning at checkpoint interrupts the expected state (hence the level end screen overlaying the gameplay). Shooting the gun then forces the level load during gameplay, and the game doesn’t know what to do from there, resulting in the soft lock. Unless the games source code is ever released though, all we can do is theorise.
