It's stated by himself in part 1: His weakness is his pride and arrogance. When he was human he took care of that to a certain extent, but the more powerful he got, the more he forgot about it. When he blinded Jotaro with his blood, he thought he had won for sure because of his arrogance, so instead of thinking, he just blindly attacked, *with his more vulnerable part* Dio's true defeat was because of his pride. Would have he thought and attacked with a punch, Jotaro would probably have died or at least been defeated. Gotta love Araki for these little details.

Dio is a chill guy when he knew someone isn't a threat to him but he won't care about took someone's life when endangered. All he thought was how to achieve his goal which is "heaven" since being the strongest doesn't make him immortal. Since he isn't a crazy serial killer, we could assume he is just a power hungry maniac but still insane enough to retain his humanity. On the other side, Hol Horse had the "dumb's luck" and Dio doesn't have enough chance to find another useful Stand User. Instead killing him, he better let Hol Horse gained a confidence againts Jotaro's group to buy enough time.

It's likely that Hol Horse wasn't expecting DIO to know he was pointing Emperor at him the moment Hol pulled back the hammer on Emperor. Stands are invisible to non-stand users along with the sounds they make being unheard. DIO asking why Hol Horse hadn't left yet most likely made him suspicious and then DIO realized what he was doing when he heard Emperor click.
