In this video Dr Janelle Sinclair talks about MTHFR treatment considerations. This is part of a video series on genetic testing for mental health. Last weeks video was on MTHFR gene mutation symptoms and health conditions. In this second video on MTHFR, Dr Janelle discusses MTHFR mutation treatment which includes MTHFR diet, lifestyle, medications to avoid and methyfolate supplement options. A methylation diet including vitamin B12 and folate rich foods is covered, as well as what foods to avoid. Methylation supplements play an important part in compensating for the MTHFR genetic mutations. L Methyfolate uses, methylfolate benefits, methyfolate side effects, methyfolate vs folinic aci,d and methylfolate vs folic acid are discussed briefly. A troubleshooting guide discussing MTHFR supplements and side effects is included. Methyfolate can be very helpful for depression and anxiety.

Thank you 🙏 I wish doctors here understood MTHFR and even know a small tid about it. I was polydrugged because of this mutation and injured by the medications I was put on never addressing the root cause. I’m trying my hardest to learn this but it’s so hard by myself. I’m also histamine intolerant so it makes it hard to eat the right foods. Please do a video on low histamine foods that are high folate. I really wish there was a cookbook for MTHFR with histamine intolerance. Thank you

Dr that hydroxo b12 with folinic acid you recommended is seriously sold out and the comments from previous purchasers says it’s frequently sold out. How can you maintain stability with a protocol this way? Hoping other brands are equally as efficacious. Btw I feel so inspired by your videos. Our lives have taken an abrupt dive because my daughter hit an all time anxiety-related high resulting in Tourette’s and the roller coaster of emotional crashes has been robbing her of her happiness. It’s the first video where someone mentioned b12 methycobalamin could be an issue. I have been taking it on suggestions of multiple ND. I stopped taking it for a few days bc I ran out and felt so much better. Thank you for these suggestions and info!!!

Hi I respect you so much. Do you know how to increase dopamine safety for AKATHISIA and dystonia? I tried lithium orotate 5mg and it made akathisia worse. Myself and others have been injured by pharmaceuticals. Please if you could help this will save a lot of lives there’s really no treatment for it except smoking pot and I’m tired of smoking pot depressed and fighting bad intrusive thoughts because of torture.

How does one get tested for that? I am an expat in Korea, and I never heard of the mutation until a few days ago. And no doctor has brought up the idea. I have bad IBS, histamine problems, symptoms of pyloruria (spelling, though no one ever tested me).

I'm heterozygous for dhfr, mthd1, and MTHFR C677T. Since these are all enzymes that convert folic acid into the active form of folate, doesn't this mean that I have a significantly reduced ability to complete this process? I am helped immensly by the bioactive form of folate, so I'm trying to figure out why, as someone who is only heterozygous for MTHFR C677T would benefit so much. Is there a funnel down process in methylation where if you are heterozygous at the dhfr stage, your ability to convert folic acid is limited at each stage due to enzymes working at half their abilty?

Because you are heterozygous for mthfr c677t it does mean your ability to process folate is decreased by 25%. I'm not surprised that methylfolate can be of benefit for you.

What do you mean by not tolerating it? I just started taking both Methyl (b12 and folate) I feel drowsy but still awake which is weird. I heard if I take glycine helps. How do you feel if you overmethylate?

We just received our order of hydroxyl B12 and folinic acid....Only to read on the package that you cannot take it with gabapentin. Is this because it will reduce the effectiveness of gabapentin? Or is it because there is a negative drug interaction between the two?

@DrJanelleSinclair Hello Doctor, I ran my DNA data through Genetic Genie (free) and 7 of 21 methylation genes have 1 variant, 6 more have double variants (MTHFR, 2x COMT, VDR, MAO-A, SHMY1). And 1/3 of my detox genes have variants. I'm disabled, l from bipolar (lucky 1%) and it's hard to spend a lot on my budget but I see you're not taking clients too. I'm wondering what supplements you'd recommend? I can send those results if you'd like too. I'd be so grateful if you have a moment to answer!

I suggest you check out the mthfr treatment report. Take note of the best B vitamins for COMT i.e. folinic acid and hydroxyB12. www.drjanellesinclair.com/mthfr Also get your vitamin D levels tested and supplement if necessary (see video on vitamin D).

Thank you for this video! I have compound heterozygous MTHFR polymorphisms (C677T and A1298C). I bought a supplement called 'B-Healthy' because it has methylated folate and methylated B12 but felt worse on it! Now I know why and what to try! I have weakened COMT, compound heterozygous (V158M and H62H). I'm so excited to try hydroxy B12 as I've felt so lost since trying methylated B12 and folate and feeling worse. I have one question, is there a reason why someone could take magnesium and feel manic instead of relaxed? I have Hashimoto's so I'm wondering if maybe it helped my thyroid too much...

I"m so glad that this video has been helpful. Did you check out my COMT video too? Regarding magnesium I don't really have a reason for you. A few people do respond that way. I would suggest that you trial a different type of magnesium. I usually recommend magnesium glycinate for depression and anxiety, but others like magnesium threonate if you can find it.

Dana Does thats my next step. Going carnivore. Seeking health has some really good products to choose from. I am very intolerant to methylfolate and methylb12. I picked up this multi from seeking health that has all of the active b vitamins including R5P, and folinic acid along with a small dose of hydroxo b12 and adenosyl b12. I’m really hoping this can help out! I have TMG if I need to add methyl groups. Good luck!

Hi Dr Sinclair, Thank you for providing such clear and helpful information! I'm heterozygous for C677T only, I was wondering about my risk for restarting hormonal birth control. Previously I've used the combined pill, implant and Depo Provera. I noticed any serious negative side effects from the implant. I'm currently not on any birth control. I was wondering if it's safe for me to restart the pill, or have another Depo Provera shot? Would supplements counteract any negative reactions or is it safer to avoid completely? And is Depo safer than the pill? Thanks!

For most people there's no need. But if you have anxiety, have comt++ or are sensitive to supplements or vitamins, then yes it's a good idea. But most people including practitioners don't know this

Anxiety en sensitive for food en supplements and herbs etc. A lit of them don't work for me, or do the opposite of what thet d o for most people. So I can imagine that I would have to use this form.

When I take folate (~400 mcg daily) I start to feel puffy (like an allergy): It feels like I'm taking too much. On the other hand taking 5 mg of extra folic acid (as required ~ 1/wk) agrees with me. Taking too too much folic acid e.g. 5 mg daily... makes me feel like I've had too much (I feel puffy): My methylation profile was tested, but folate levels are normal so the folic -> folate conversion is working. Maybe higher than normal folic acid is all that is required to push the reaction to the right? My doc has suggested I try hydroxy-B12 (currently on methyl-B12)... which I like, but it makes me feel puffy (like an allergy) if I have too much i.e. 2000 mcg daily. Is hydroxylation preferable to methylation? I'm homozygous for the MTHFR (C677T) mutation and heterozygous for COMT (only one allele inherited). 🙂

I would usually only recommend 600-1200mcg (0.6-1.2mg) of folate. 5mg is a lot. When you say you are homozygous for mthfr, do you mean homozygous normal/wild type or homozygous for the mutation?

​@@DrJanelleSinclair I only took 400 mcg of folate. It is folic acid that agrees with me in higher amounts i.e. 5 mg (prn). My homocysteine has been normal for many years... suggesting that the folic -> folate conversion is occurring just fine. My doctor found this surprising... which is why we followed up with a methylation profile to check. My daily B-complex contains folic acid but if I flush (very mildly) when taking slow release niacin I take extra folic acid (5 mg) i.e.Normally you don't get any flush with slow release niacin (inositol hexanicotinate 500 mg). Folic acid is the rate limiting step in niacin metabolism. If you get a flush it means you are a bit short of folic acid: The flush disappears as soon as you take extra folic acid. MTHFR is homozygous for the mutation. 🙂

Folic acid is the synthetic form of folate that is found in some supplements and fortified foods. Natural folate in foods is not in the form of folic acid. So you don't avoid natural folate.

Dr Ben Lunch recommends that people with the mthfr genetic weakness eat gluten free. Eating gluten free is definitely preferable especially for anyone with poor mental health to help lower inflammation in the body. But as far as I know there's no interaction between wheat and the mthfr gene or folate.

The supplement I take contains the following: 400 mcg of folate - 6(S)-5-methyltetrahydrofolate, 5 mcg of vitamin B12 cyanocobalamine Should I get one with methylcobalamine or another kind? They created it specifically for this but I still have depression regardless...and feel like I am not at my best health-wise. Thank you, I am homozygote.

It's usually recommended that if you have mthfr genetic weaknesses that you take much higher doses of methylfolate. I.e. more than one capsule. I'd also suggest that you do take another supplement with higher methylcobalamine.

why would you want to eat foods that are high in folate but avoid foods with folic acid?

If you don't methylate correctly, folic acid will be toxic. Folate is more bioavailable, methylfolate is even more bioavailable where as Folic Acid is a synthetic vitamin you cannot breakdown.

That's the medical model treatment for MTHFR. But if you have the slow COMT enzyme it's likely that this dose may increase anxiety and anger. If you're homozygous for COMT rather than homozygous "normal" for COMT you may do better with folinate and hydroxyB12

@@DrJanelleSinclair I am slow COMT as well as MTHFR Homyzygus for c677. I recently lowered the dose to 7.5mg methylfolate after learning this. I’ve been taking 15mg for years. I do have some anxiety but, never noticed Initial changes when starting the 15mg. I wonder if I should stop it and try the hydroxy b12 / folincic acid your recommend in your COMT video. I just want to make sure I can still support my methylation issue. After taking 15mg for 4 years should I taper down?

I just got my genetics results. I have c/c (C677T) and c/c (A1298C) and also COMT genotype A/A. Should I try supplementation? I have pcos and have always struggled with obesity and anxiety. I had preeclampsia in my first pregnancy as well and I am afraid to have another.

Yes that's a detox effect. Perhaps reduce dose for a while and try some electrolytes including potassium

Only read if you or someone you know has overmethylation. If you have overmethylation then avoid all the fish, meat, liver and eggs, they are incredibly high in methionine and will worsen your symptoms very quickly. Methionine is a methyl donor and you don't need more methyl groups with overmethylation. Pretty self explanatory. You can try for yourself but I guarantee you will feel much worse until you stop and go on a mainly vegan diet. Also incorporate glycine, overmethylation has a lot to do with glycine deficiency as that's the only natural way to lower methionine from the food we eat. 3 grams before bed on an empty stomach. And possibly supplement b3 as niacinamide as that lowers serotonergic and dopaminergic activity and in order to metabolise niacinamide you need to methylate the metabolites to pee them out thus reducing methylation. The only thing you have to watch out for while supplementing with niacinamide is an increase in homocysteine levels. Methionine is the main methyl donor used to methylated and excrete niacinamide. And when you strip away the methyl group from methionine you are left with homocysteine. I was taking a lot of niacinamide and after a few weeks started getting sores in my mouth and my skin started flushing and that would subside about 2 days after stopping the b3. My current stack: 1,200mcg Folinic acid (unmethylated activated natural folate) 2,000mcg B12 as hydrox/adenosylcobalamin Glycine (safe up to 50g+ daily) Niacinamide 1,000mg 5x a week The folate and b12 works to lower homocysteine so whenever your homocysteine levels go up too much just stop the niacinamide and any symptoms should go away pretty fast especially with higher doses of both b9+b12. B6 also helps convert homocysteine to glutathione, our main antioxidant. All of what I just stated is for people who overmethylate only. It is hard to find information on overmethylation that's why I took the time to help you now. You can verify everything I said, it's 100% correct. To sum everything up, avoid high methionine food. It will worsen your symptoms pretty fast. Supplement with natural folate, if you can't handle methylfolate then try folinic acid. Supplement with b12 as hydroxocobalamin, or hydrox/adenosylcobalamin mix. Take glycine, 3grams before bed on empty stomach. Eat plenty carrots/vitamin A foods as they're required to make the enzyme that glycine uses to lower methionine. Supplement with niacinamide also if you have severe overmethylation like me, but watch out for high homocysteine. You can get it chacked but you should know when it goes too high as your skin will start randomly flushing hours after taking it, and you'll start getting worsening mouth sores until you stop. Also consider taking b6 either alone or in a multivitamin (I take multivitamin). It is required to convert homocysteine to glutathione the master antioxidant. Why would you want to reduce the body’s natural ability to properly methylate. There’s not a genetic defect that causes “over methylation” is there…???

Everyone in my family that has tested positive for the mthfr mutation is A- including myself and my daughter. Our family is huge too. Granny was a bit old fashioned lol.

@@Truerealism747 i actually have Asperger’s! So do many of the “younger “ relatives. I say younger because of misdiagnosis decades ago. My youngest and oldest are O- and O+ and are completely different from myself and my middle kiddo. I never thought blood type had anything to do with personality, but wow. I do now lol.

Dissapointing to see your telling people to avoid vegan/ vegetarian diet. People who are vegan won't add meat as it is an ethical stance. Beans and legumes are a large part of the diet. B12 is routinely given to the animals feed so may as well take b12 supplement rather than have it filtered through the animal. Other than this criticism, I really appreciate your videos. Very educational. Thankyou

Thanks for the feedback. I understand that for some people a vegan diet is about the ethical stand but for others it's for their health. We can all have different opinions but I feel that it allows others to go and do some research. It's good for people to know that risks and compensate for them.
