Very informative. I am very far sighted (+5, +6) and am waiting for the technology that can get me out of the discomfort of contacts and inconvenience of glasses. I think it is awesome that you and your team take the time to educate the public on topics like this!

Glad you liked the video! Yes, +5 to +6 is on the high side of farsightedness. You could check with your doc but when it is time for cataract surgery, multifocal lenses may be an option for you.

Hi Dr. Porter, where do accommodating lenses fit in? Are there any disadvantages?

Sir please tell me that which IOL would be best for a one eyed person who have Retinitis Pigmentosa in cataract surgery? He has 6/24P visual acuity in left eye and right eye have no perception of light. Can he choose multifocal IOL. Does there comes a IOL which have good near+intermediate vision and patient have to wear spectacles for distance vision ? This patient age is 32 and he is involved in Academics and Athletics and other sports like football, cricket etc.

Dear Doctor, I am a seventy-one-year-old retired American man, living in Ho Chi Minh City (Saigon), Vietnam where I tutor English. I need your help regarding my choice of IOLs for my cataract surgery. In 2000, I had Lasik surgery and because of my astigmatism I was given mono vision treatment. That is, I see distance with one eye and up close with the other. Lasik correction for astigmatism was not common at that time. I currently use reading glasses for small print or extended periods of reading. I have distance sunglasses for outdoors but rarely use them. Recently I have stumbled and fallen partly due to lack of contrast at night in dim light when going down steps or crossing a street with potholes. I read, write, and research on computer most often. Now I am not sure what would be the best IOL for me, accomodative, multifocal, or monofocal. Which lens would you recommend and why? . Any recommendations or advice is greatly appreciated. I'm a truck driver I have a prescription of 7.00 + and 8.00+ in the other eye I need a lens more for driving then reading !! what lens would you recommend

what is the diffferent between monofocal lens and multifocal lens. my insurance does not cover multifocal lens. what are my choice.

+Joe Tran The multifocal lens can give vision at all ranges (near, intermediate, and distance) without glasses. Monofocal lenses only are focused for one distance and require correction with glasses or bifocals to obtain vision at the other distances. With monofocal lenses, some patients choose monovision where one eye favors distance vision and the other favors near vision, in order to obtain a full range of vision. Hope this info helps!

Thank you Dr. Isaac..one more question..because the insurance does not cover multifocal lens. If I go with monofocal lens now..and in the future, a few years from now and I decided to change to multifocal lens..is this possible.

It is possible to exchange a lens implant, but it may not be easy to perform and there is a risk for complications. The lens implant is intended to last a patient's lifetime. In the future, there may be other options of adding an extra multifocal lens into the eye (in front of the monofocal lens) to obtain a full range of vision. This may be a more straightforward option if it becomes available here.

I am super confused about my choice of lenses. I am early aged(28) cataract patient and I am academic researcher, I am visiting eye specialists they offer me to have surgery now. they speaking like monofocal is better, multifocal has their own problems. I really want to do best, I don't know what to do. I am from Turkey but I live in Czech republic. I wanted to have it in Turkey but I am not sure about choice

The best choice varies from patient to patient based upon their goals. Multifocal lenses can give a full range of vision but do have a small tradeoff of decreased clarity. Would discuss your options with your surgeon to see what's best for you.

+Isaac Porter, MD thank you sharing your experience and information with others. You can even reach people who are far away from you. Such as me. I almost decided to choose multifocal lens. I don't want to use glasses after operation. I will meet with my doctor tomorrow if he still try to convince me to monofocal lens I will see another doctor and ask his opinion too. Or maybe wait one or two years more. maybe different kind of lenses can appear.
