It's not a mistake, despite being wrong. A lot of roman numeral clocks have IIII as 4. I assume it's to avoid confusion between IV and VI, since they are both pretty much upside-down at this point on a clock. I have a clock at home with the four sticks like the one seen here.

Its not a black eye, i dont think. Its based off of the movie Clockwork Orange, and though i've never seen the film i do know one of the characters has that marking around his eye and that is an identifiable trait of the film. if you just Google image search Clockwork Orange you can see it.
