To answer the question about whether the Glyph of Balance guarantees the room clear drops, I believe it does, I might be wrong on this one but I think it modifies the times when the clear would normally not give anything to instead function like the Glyph drop and when the room clear would provide a reward normally that reward stays unaffected by Glyph effect. Also I heard it has some interaction with hard mode exclusively where it can make a room clear not give anything because hard mode has a 33% chance to replace a heart drop with nothing but I’m not sure how that works or how rare it is.
