R key is like the D6 except (most of) the items you reroll grant you their benefit and are taken away from the item pool forever when you "reroll" them. Both items "suffer" from the "problem" of picking them up late where you've already won, but runs where you're running out of steam will only be saved by the R key and very likely not D6. R key is def higher than D6

I couldn't quite rank Ipecac better than the fetuses. Ipecac is a good item, but it doesn't like synergies much. Once you start getting multishot or fast tear delay it becomes extremely dangerous. Definitely don't recommend Ipecac for Lost (or TLost) runs. The vast majority of the mobs charge you head on. Even if you're super careful with your shot placements, it's only a matter of time before mantle gets hit and then they run into you and kill you. Edit: It does like synergies, but basically every synergy for Ipecac is going to require you to either have explosive immunity or some way of mitigating the self damage.

@hollowknightenjoyer Dont correct me since I'm not wrong, but it doesn't do that at all. I have used it before and can confirm that it sends you to a floor where you can get any item in the game. Then brings you back to where you were when you pick it up. Death certificate is quite literally the best item in the game by default.

Dr. Fetus is so good because it has so many broken synergies. I got an insane run where I started with pyromaniac and a curse room with curse of the tower. Fast forward to floor 3 where I got Dr. Fetus and made it a goated run

IMO book of shadows should be A at least. There is a lot of characters for which it is insanely powerful such as the losts, tainted jacob, bethany, tainted maggy etc. It allows you to abuse health gambling machines (most notably blood banks and demon beggars). It has insane synergies with any damage on touch/orbital as well as proctosis and book of virgues. It can safe your run when you had bad luck with heath going for something like delirium. Insane value from pairing it with any active item buffers. And quite common to boot.

I am comig back here and I can't believe you put kamikazi in F tier, have you even seen it with bethany, or T.Maggie. you can even use it to save your deal chance if you know you are about to take a hit. It is situational, but in those situations it is broken. I got it late in a crappy low stats Mother run with high health as bethany, and it carried me through the rest of the game easily with no effort.
