Yeah sounds like that place was understaffed. That’s way too many patients for one nurse. That’s how it is throughout nursing for the most part. Waiting for the day there will be laws in place about nurse to patient ratios.

Usually if it’s 6 babies you won’t have any moms but you can definitely in certain cases have four babies and two moms

I always had a minimum of 4 babies and 4 moms. The maximum I was supposed to have was 6 and 6. It didn’t always work out that way….

As always, hospitals always choose to have profits over patient safety. This is one one thing I've always hated about the nursing profession. However, one thing I do appreciate about California (specifically Northern California) is that our nurses unions are very strong and always push for patient safety, which means administration is forced to listening to, and act upon nurses' concerns.

She didn't lie at all here. Postpartum nursing is a different world. It is a lot of work. And we everyday because we often get compared to other hospital areas in terms of the number of patients. Everyone wants to believe that it isn't a tough job and that everybody is haply and healthy. Nope. Happy she found her way to the NICU where she desires to be. Only thing I caution people on...please do not use postpartum solely as a stepping stone. I caution you on this because this can lead to bias and can negatively impact the care you provide the mother baby dyad (not saying this is her case ). So like she mentioned there were days when she would just feel like she is reporting to work but she didn't care to be there in that particular role. Her mindset could have easily caused her to look over or dismiss critical aspects of her patients care that can affect the mother and her family long term.. Her nursing experience and beautiful disposition arent enough to maintain sound quality nursing care. You have to have the desire to want to be not just good in this field but to want to excel in this field and make the greatest impact in this field (again, not saying this was not her case). Please seek nursing entry programs, field transition programs, or simply talk to your manager about helping you develop a professional development plan that will best help transition you into a new role in the future. Sometimes, your manager may be able to meet with the Manger of your desired department to help you. Yes, I know that every Manger isn't helpful or nice in which case this may not be the route you take.

Thanks for this advice. I just applied to my school's nursing program, and was wondering as to what fields or departments I would like to work in. I didn't want to 100 percent commit to one department forever, I would like to try a few different ones. I wasn't aware that field transition roles existed, so thank you for this insight!

@@sethsha7826 best wishes in school and do all you can to enjoy. Yes such programs do exist. Be mindful that this may be larger hospital systems more so than smaller hospital systems. As you approach your senior year, consider looking for internships. Seek out volunteer roles that get your feet in the door of the facility. Things may be different in light of the pandemic, but if things change they may become available again. And to clarify what I mean by transition...we have a program that we do once in a while that allows nurses working in other areas such as telemetry, or ER and we train them in areas like Labor and Delivery or NICU for a 6 months or so. Hope that makes sense. Take care.

I’m a mother of 3 kids, in college for Nursing and I agree 100% with you. I don’t know what were the goals of the 2 nurses that I had when I had my first child but I HATED THEM to my core. They FORCED me to pump my colostrum while my nips where raw bleeding, refused to feed my baby with formula and they were pissed because I wouldn’t latch my baby. He couldn’t latch because MY BABY WAS TONGUE TIEEEED. And the so called LC RNs never noticed his posterior tie. Only the NOC nurse was compassionate and told me that I could just exclusively pump and that was okay. Fed is best. But honestly the worst experience of my life in postpartum care. For my second and third baby I was prepared, I brought my own pump, my own formula and bottles. I was shocked how different the care was compared to my first child (different hospital) both girls were also tongue tied but they never harassed me when I decided to formula feed until their ties were clipped.

U are 100000% correct. Nursing is definitely not a career where u just go for “the money”. U have to want to be there bc you can easily kill someone. I am a nurse myself in pediatric hematology oncology and I can tell you that I miss going on break often, have to hold my pee in, get yelled at, cry and the whole nine yards bc it is such a tough area in the field.

i just started as a postpartum nurse this month and it is so much more than i expected! we typically have 4 couplets and you’re definitely running around like crazy the whole shift. i love my job so far but the patient ratios are rough and when you say you take care of 4 couplets people sometimes don’t really realize that is 8 patients and makes for a very busy day, even if our patients are typically healthy

I love that their is so many different departments that you can work in as a nurse and don’t have to stay somewhere that you don’t seeing yourself working forever and ever and you can always switch to another department.

I appreciate that you shared your experience with the mom/baby unit! I am a new grad who started in the mom/baby unit over a month ago and I will admit it was soooo overwhelming at first with all the charting and tasks that have to be done. But now that I am adjusting and feeling more competent in the role, I am really loving it. I just want to encourage other nurses that may be starting in this area that while challenging, it can be a really good unit!

You have the cutest, best form fitting scrubs ever! I seriously struggle to find scrubs that hug the body parts that I want it to, and to be baggy in the body parts that I want them to be baggy in. Can you make a video, or a scrub haul on scrubs? Drop the brands, fave stores, etc. Thanks!

The charting at the bedside is unreal! I work in endoscopy now and there is a lot of charting but the patients are mostly stable outpatients (although we do get inpatients too) so I don't mind as much focusing on the charting and less on the patient. At the beside I often felt like I had to choose either to get my charting done right or do the nursing part right, and that the hospital was pushing me the wrong way and only cared about the charting. Made me sad and frustrated. I'm glad to hear you are enjoying NICU!

I definitely agree with you. I was an adult surgical/orthopaedic inpatient nurse for 2-3 years. Realised it just wasn’t for me and felt it was getting repetitive so now I’m starting Paediatric Emergency this May! Really excited for something fresh and new but still be able to stay in the hospital. Good luck in NICU 👍

I'm 6 weeks into postpartum nursing as a new grad and girl! I'm drowning!!! The tasks the CHARTING 😱 The best days are when we have a nicu nurse to come care for the babies and we can solely care for mom but that's only when we're short staffed.

Unsolicited advice: Go to med surg for one year - just a year. You’re not going to be a well rounded nurse from postpartum and you’re limiting yourself for your future career. If you can do med surg for a year, you can go anywhere from there, including postpartum.😊. You don’t learn any nursing skills on postpartum, like central line care or even starting an IV. If you’re like me, one of the last places you want to be is med surg, but the experience of learning WILL prove so beneficial to you as a nurse. 😊

@@lulub9421 thank you for the advice! Almost 4 years into nursing and halfway through FNP school. I’m the one everyone call to start IVs and thankfully my floor cares for GYN patients so we do get a good bit of med surg skills here and there.

I love postpartum nursing. I never have more than 3 couplets. I was a pediatric nurse for 2 years and I have never been more stress-free in my nursing career than I am now. I guess to each it’s own & depending on the hospital, it can be harder/stressful. Good luck in the NICU! ❤️

I did my immersion/capstone on a mother baby floor and I literally love love loveeeeeed it! I woke up everyday excited to get to the floor and help mommas take care of their new babies. I honestly really wanted to continue on and work in mother baby as a new grad but I got offered a position on a pediatric surgical floor and felt that as a NEW GRAD I should take the “harder” floor. I felt very comfortable in pp and like I could manage everything I needed to, I just felt that I had so much more to learn so I took the pedi offer. I just finished my first week of residency on the floor and I am feeling so so so overwhelmed. I know it’s just growing pains but I’m hoping my pedi floor starts to feel more manageable..

Lucky you no more than 3 couplets. Must be in in Cali. Here in Central FL it's like 4 couplets to 1 sometimes 5 because everyone is moving here and its getting extremely busy.

You are not lying about the charting! The charting and the protocols for mom and baby is just soo much. I personally love postpartum but it’s my first job out of nursing school so it kinda all I know.

I started out in postpartum and I finally got into the NICU in December. I am so happy to be in the NICU now. I actually really did do well as a postpartum nurse, I just felt like I was craving more when it comes to skills etc and I hated the feeling of being scared when something new would pop up because you only deal with the same few things over and over again. I would still gladly float to my old floor though, especially when I want a little break from the more critical babies. I am glad to have had the experience because it helps me interact with the NICU moms so much better.

I really needed this because I am a senior in high school enrolled in a CNA course; nursing has been my goal. After going to clinicals and disliking nursing homes, I recently started questioning whether or not nursing is for me - changing briefs and moving people all day is definitely not something I want to do. This video, however, is encouraging me to stick with nursing because it is different between fields. I would love to be a nurse anesthetist or nurse practitioner in the future and I know I will need to, for example, work in the ICU before getting into CRNA school. I feel more motivated now to continue on my nursing journey even though I will have to go through things I dislike because it is such a diverse career and I am confident I will find my niche. If you are a new grad rn going into any unit there will always be documentation and a lot of it. Taking care of a patient or couplets is always challenging. Nursing is challenging where ever you go. That's why learning prioritization and time management is highly recommended especially for new grad nurses.

I've been a postpartum for over 16 years. I love it. I love the couplet care. The charting will always be my least favorite thing. I found charting as I go as long as it is feasible (a mom or baby isn't having issues) works well. Everyone should follow their path. And I've worked with nurses who solely used postpartum as a stepping stone to Nicu and L&D. The only thing I hope is they have gained an appreciation for couplet care. Just like I respect those nurses in those fields and appreciate them. I would never want to work in L&D nor Nicu so I appreciate those nurses who want and love to work there. I'm glad you finally are in your dream job.

I want to work in postpartum so badly... I started in L&D and only survived 6 months, it was so scary and stressful, I didn’t even pass the orientation... I’m still trying to find a new job and wish I could be in couplet care. I wanted to start there or NICU but couplet care never has openings. People like it so much that they never leave, so there’s never any openings 💀

@@b.freeman2118 congrats. Sorry I just saw this. I would just learn as much as you can. Always ask questions and be an advocate for your patients. Be open to various viewpoints of your patients for care for themselves and for their new little ones.

Right like idk how she did it. At the hospital I worked, you have to stay in an area for 6 months before they let you transfer to another unit. I tried to transfer right before my 6 months were up because I wasn’t performing so well, but I didn’t make it.

omg 4-6 babies is crazy! I work as a postpartum nurse too and we have 6 patients total maximum (so could have 2 nicu moms & 2 couplets or various versions of that), I can't imagine having any more than that!

She’s right about the ER charting. I work pcu and whenever ER send pts up to the floor we hate it, but when I get floated down there, I see why they don’t do crap ! That’s because THEY DON’T HAVE TIME. Pts in ED are literally in and out, unless the entire hospital is full

Oh wow girl! I just started in postpartum mother/baby and we do couplet care. We have usually 2-3 couplets but NEVER more than 4. I’m a new grad and only a week in to my residency but so far I’m enjoying it. I know what you mean about all the charting though!

@@Mariana-hq4ug I enjoyed it! I did it for about a year but now I’ve been an L&D nurse for a little over a year and I’m in grad school to get my DNP in midwifery. Mother-baby is great, I was just more interested in the delivery side.

I had my clinical today at MIU & i didn’t like it either 😞 I’m happy to have seen what it was like though, since in covid we aren’t able to much clinical. i’m glad i got to know from today that MIU is not for me either :/

At my hospital it is a separate job. The mom nurse and baby nurse. The nursery handles everything baby and l&d handles mom. They literally had you doing two jobs in one!

Same I’m just the baby nurse and it’s not bad. I came from medsurg and it’s much less stressful, a different stress though depending on what’s going on. I’ll take babies over adults any day though!

I work in a unit where our NICU is attached to post partum. Occasionally we are floated to the post partum section and I HATE IT. There is no ratios, it’s ick.

You have to be passionate about your field. The heart wants what the heart wants. I love babies, I love women, I love teaching, I love breastfeeding. I’m passionate about this world because I have 4 children and my hospital experience is why I became a nurse. I’ve been in cardiac stepdown and med surg for almost 5 years and absolutely hate my nursing life. I do not care anymore like I should because I’m burnt out on doing something I’m not into. It will kill you if you stay in that mode too long. Fortunately, I am going into mom baby at the end of the month! 🎉❤🙌🏽

Strangely enough this sounds super similar to when I worked in the infant room of a daycare. 4 babies to 1 teacher, all in the same room, and had to “chart” all day long *feedings, diapers, constant flow of pictures to parents, etc. One of the most difficult things I’ve done is try to put one screaming infant to sleep while three other infants are screaming next to me. All while pulling in a whopping $12 an hour. When infants are healthy people think you can just take care of a bunch at one time but you CAN’T. Especially at $12 an hour 😒

I had the sweetest PP nurse after having my baby a few months ago and I had her again recently when I was admitted after having surgery. She was so kind and caring she's got me interested in becoming a PP nurse. I don't think I could handle being a L&D nurse but I am kinda interested in that too..

I left CICU ( my dream job) for a PRN sedation nurse in MRI. I did so because I needed the work life balance. I didn’t love it but I worked with amazing nurses and didn’t do nights, weekends, or holidays. I hung in there for 9 years but about a year or so I felt like I could no longer do it. The environmental was becoming to toxic. I took a job as an IR nurse within the same hospital. Again I really wanted to love this job but I am so bored most days. I’ve wanted to return to PCICU for many years but didn’t want my family to suffer.( mom of 7) I’ve been there for almost 4 months and I am trying to stay for at least 6 months because they are trying to fill a FT position. I don’t want to leave them stranded because they are a wonderful group of people to work for. however a CICU job came up at a hospital much closer to home and I really want to apply. The only other downside is that it’s been 10 years and it would be like learning this all over. I am ready to finish out my nursing career where my passion has been.

I'm going on day 3 and I already feel like its not for me! The charting is ridiculous! I I hope it gets better. Also, they don't use Epic and I'm use to epic. I'm trying to be in a mindset where I don't compare it to my previous job because this is defiantly harder than dialysis. Feeling so discouraged :(

In NICU I’m certain there is a lot of paper work but you won’t have that many patients. As far as the mother baby they come in pairs, so while your dealing with one your dealing with the other at the same time but both of them need different things.

I’m sooo late to the party but I want some nurse opinions. I currently been working in med surg for 4 months. It’s a med surg/COVID unit & our ratios are 6-1… in-FREAKING-Sane! I chart soo much but I’ve gotten used to it now & I’m a little quicker than I was before. However my heart is in mother/baby nursing/ NICU. I’m curious if anyone has transitioned from Med surg to mother/baby? What was it like? Were you overwhelmed? Thanks for the advice if I get any

just kno you're probably going to have just as much charting if not more in mother/baby bc they give them higher patient ratios (depending on where you work of course). you might have 4 couplets (4 moms and 4 babies) so that's 8 patients to chart on and you have several discharges to get done every shift guaranteed since several moms will be discharged everyday unlike other units where you may have no discharges in a day. so lots of assessments, discharge paperwork, and admissions to replace your discharged patients

“The mom was crashing or the baby was crashing”. What kind of postpartum unit did you work at? I worked Postpartum for years and never had a mom or a baby crash. The charting doesn’t need to be a long narrative. I’ve seen nurses write novels unnecessarily. You need to chart, yes, but I always reserved my novels for any abnormalities, which was rare. Hospitals would do well to have all the charting at bedside, computerized (no questions about times and required information). Great video, happy nursing! 😊

In Germany, we usually habe 10 mother’s plus their baby’s per one nurse, I had nightshifts with 19 mothers and their baby’s, it was a living nightmare

I work as an LPN on my mother baby floor. Take my own patient load - 4 or 5 rooms so up to 8-10 moms and babies. I know not all areas in the US don't use LPNs in post partum but in Canada (Sask) we do!

Great video! I’m in step down rn and I’ve always been interested in mother baby, loved hearing your experience. I’m a nurse in California but interested in moving to Texas. Can anyone in the comments give me some insight into what’s it like being an RN in a state with no patient ratios?

I’m not in Texas, but I used to work in GA. It’s not that there aren’t patient ratios. But they don’t have a union to enforce safe ratios. I did adult pulmonary nursing and our ratio was 1:6. However, if the unit is short, you very well could have 7. I hated it. I’m now in CA too, working in the NICU.

I’m a Texas nurse. Really depends on what type of nursing you do. In my experience... med surg 1:6 maybe 7 on a rough day, tele 1:4-5... I’m a preop Pacu nurse and it’s life compared to working on floor. I did most of my time on the floor, learned a bunch but not sure if I could ever go back. I started out on a trauma step down in a level 1 trauma hospital.. rough days were almost daily!

As a RN in Texas AVOID HCA hospitals, I don’t work at one, but i know people who do. They send nurses home as soon as pts get sent out and then the ratios get skewed bc they get more patients that the rest of the nurses have to take.

I read about on reddit some couple in texas that moved to California for the nursing pay and ratios. They said they liked living in California a lot more because of that. I honestly would work for a while in California and then buy a house in cash in Texas or at least a large downpayment because the salaries are so much higher here.

Mother baby ratios in Houston Texas are usually 2- 4 couplets max unless you work for HCA facilites.. you will more than likely have what Alexis mentioned on this video. Did you come to Texas?

Totally felt this in OB rotation now I was taking care of my mommy who had a c section needed pain meds and assistance with restroom and then baby on the bed rail down lord mercy she was holding on to me while my other hand was on the baby so he doesn’t fall and he was screaming bloody murder cause his mommy was moving away from him. I don’t like it at all so I see what you’re saying two different fields in one. Not my fave

In Russia there are 4-6 patients in one room at the hospitals! Every mom with a baby. That's where it gets complicated!! 😅😅

i feel like every nurse thinks mother/baby or postpartum must be a chill unit if they haven't done it before but they give them higher patient ratios so it's a whole lot of charting. as a pca, i'll be walking moms on the mother baby floor after their c-section and we'll pass by the nurses station and no one will be there bc all the nurses are busy and in one of their patient's rooms and people will ask me are you the ONLY one here? lmaooo no i am not, it's just that all the nurses are busy bc it's too many patients for one person. so i really don't think i would like mother/baby as a nurse. don't go to mother/baby thinking it will be a cakewalk just bc the patients are less critical

A midwife is basically a doctor, they have gone through certain education requirements that allows them to perform certain things that a doctor can - like C-sections. A midwife is often the primary care provider for a pregnant patient, following the person’s pregnancy and being the one to deliver their baby via vaginal birth or C-section. A postpartum nurse is a nurse who cares for people after they have already given birth. The baby is out already. The nurse receives the patient from a labor and delivery nurse (or from the OR staff, if the mom had C-section). Postpartum nurses don’t take care of pregnant people. Only after the baby is out. They make sure mom is recovering from the birth, make sure she isnt losing too much blood, make sure baby adjusts to external life well, make sure mom and baby are healthy & bonding before they go home. Basically a midwife is like a doctor, who takes care of a person during pregnancy and labor. A postpartum nurse only cares for people AFTER birth, before they go home.

@@Crazypeople246 I’m not sure where you’re from but here in Canada midwives cannot perform c-sections! Midwives are technically providers (like doctors) however they cannot have any high risk pregnancies, multiple gestation, c sections, etc! Only vaginal births :)

I should add I love In Australia. And I’m an RN not a midwife. To do labour and delivery you have to be a midwife. I’m definitely a NICU nurse and not a midwife hahaha

As a postpartum nurse, you shouldn't consider the baby and mother as two patients. The baby and mama are one unit together and should be treated as such. Also, excessive charting is a part of every nursing job no matter what specialty you're in, there is no getting around that.

I found post partum nursing hard. I got to the point were I was really fed up with the women taking on the sick role all the time. Lots of well women buzzing for you to feed their baby because they are tired. I get it if they had had surgery/ sepsis etc, but this generation are very lazy and don’t want to take care of their babies. I was more like a handmaid than a nurse
