Dr. Sharon, some of these questions are on Uworld. I will need some clarification because Uworld has the exact same Levothyroxine question word for word and actually says that the skin being cool and dry is NOT something you should expect after treatment. It is saying dry skin is a characteristic of decreased blood flow in hypothyroidism and that the answer should say that the skin should just look normal. It's difficult when there are small inconsistencies like this between teaching platforms, I apologize. It makes it difficult to know exactly what answer NCLEX would actually accept.

I agree here. I also remember in the Kilmek reviews Hypometabolism (another way of looking at hypothyroidism) is all low. So having cool and dry would mean their symptoms aren’t improving since those are symptoms of low. An improved would be normal like u said

That’s the same exact question on UWORLD and have done it without including skin is dry and cool and I got it right.

I hear you. I think this could go either way. Those with hypothyroid are COLD, so when their thyroid is normal, they feel warmer. I suppose the reason uworld says this answer is “wrong” is because of the word “cool”, I suppose they would say it should be “warm and dry”, but I think that is too narrow of a distinction

for question 2- my first thought was to not pick D because we want warm skin that to me is normal not cool skin

Hi! I'm stumped on the second question regarding selecting D. I chose not to select it because the hypothyroid patient already has cool and dry skin, so in turn when the Levothyroxine is correcting the hypothyroid symptoms, I assumed this to be incorrect since that was already an expected finding of hypothyroidism. Could you offer some clarification on this? Thanks!!

Hello Dr.Sharon. I am just coming across your videos and these sata questions and how you break them down is really helpful. I have a question about question number 5. You said you are not worried about the BP is 140/84. can you explain why because I thought to pick it because it is high... thank you

Hi! Their systolic is elevated, but their diastolic pressure is within normal range. 140/84 isn’t a huge concern AND they are prescribed clopidogrel anyway which will thin their blood anyway and in turn, help prevent clots with a slightly elevated BP so it’s not concerning. The question is asking what data would cause harm to the patient if they took this med. Therefore, the BP being slightly elevated will not have a concerning/deadly effect. Hope that makes sense!

The question regarding hypothyroidism, you included skin is dry and cool as one of the answers and on UWORLD, it doesn’t include that answer.
