Josue Amaya the manga never referred to pitou as “he” or “she”. Gon, Killua and the rest of the team called pitou “it”.

Neferpitou, officially speaking, doesn't have a gender. In the original manga, only gender neutral forms of address are used, which is much easier in Japanese than English. Being a chimera ant and a royal guard, Neferpitou is probably not capable of breeding one way or another, so the only gender they would have would be derived from whatever chimerical influences when into Neferpitou's body. While in the case of the other royal guards, and many of the lower chimera ants, these give a "definate" gender, in the case of Neferpitou (as well as several other lesser ants) the genetic influences going into them were ambiguous, and therefore Neferpitou retains the default genderless state of a non-breeding individual.
