This series of talks by Dr. Lee is EXACTLY what excellent teaching is all about. No need for lasers, videos, slides....just someone who really knows their stuff and how to explain it/make it interesting with a board to write on. I always thought some of my own best talks were done this way but since finishing residency/fellowship I rarely am privileged to see someone do this (or teach this way myself unless it's one on one) Deep respect for Dr. Lee here! I'm going to watch all of these and make myself a better Ophthalmologist!

I was diagnosed with IIH. My papilledema was bad. Really bad. Really bad "cluster-like" migraines constantly. Dizziness. Pretty much a constant hypertensive crisis (resistant hypertension). I'm not joking when I say I said "well if I go blind it is what it is." I stopped going to the doctor and started dieting and exercising so I could return and have her consider me as a person and not as my weight. Lost 60 pounds. At my lowest weight of 190 (I'm supposed to be about 170) I developed tachycardia (150 laying down, 190 standing up), dizziness, left sided ventricular hypertrophy, and reduced EF. No supplements. Normal everything else and papilledema was nonexistent but sever migraines remained along with double vision. Come to find out I possibly have multiple autoimmune diseases, am severely anemic, and have POTS. Due to my inability to stand up in general, I gained my weight back VERY rapidly, and my papilledema did not return. Headaches remained. I think that there sometimes is a confusion between correlation and causation. I've been unresolved on my health issues and the cause of my eye issues and IIH and heart since I was 16. I'm 30 now. Sometimes there are underlying conditions that don't help. And to any doctor out there, please listen to what your patient is telling you. They're the ones that live in their body. I know if it wasnt for a doctor that listened to me, I would probably have had severe complications due to kidney issues (bad reaction to diuretics) and I wouldn't even know my body is being ravaged by autoimmune illness. Because a doctor listened to me, I am now taking steps to get help so I can hopefully improve my quality of life and maybe even lose the weight again. Who knows? A doctor listening is sometimes all that takes to change the course of someones life for the better.

Thank you for explaining, I was diagnosed with bilateral Papilledema grade 3 today . It makes me feel better to understand what is to come.

Just got diagnosed today. My issues began in 2005, I used to have these awful headache in the back of my head. In 2014 as a student at Stony Brook University, I fainted from the pain, and was taken to the SBU Hospital. They did do take a spinal fluid sample that day, but I was not told the cause of the headaches. So this is what I expirience: My brain's arteries and veins biggins palpitating with my heart beat. Now, as a man, the headaches and symptoms appear most when I have sex, as I start at the ejaculation phase it goes higher and higher, and at the hight of the pain, my brain feels as if is going to blow up. Is reall, really painful. The headaches just began to reoccur just as my peripheral vision on my left eye became wavy as well (this might be due to retina detachment, and tomorrow I will go to a doctor specialized in retina), I see blurry and flashes of light. So right now I cant even have sex. This sucks. Doc can you tell me what to do so I can go back to my normal routines. By the way, are advils okay to take when experience this pain?

Thanks for the information! For obese asymptomatic young female with papilledema, I’d refer her for an urgent CT scan, alert her the worse scenario tho it’s unlikely. Your practice will be protected only if your patient is well informed and shoulder the risk, or you will face the risk alone. In reality, some patients won’t follow your advice anyway because they don’t feel any urgency about it. That’s okay. Because they feel they know their body, they are informed and they are exercising their autonomy on their free will. That’s a fair game. But, if you tell them there’s nothing to worry about it, take your time for further work up, you will become the one who goes to home with finger crossing.

How long should u take acetazolamide? I was diagnosed with papillidima and I had stop the mediation for some months now I do get headaches and eye pains sometimes and I'm working on weight loss

Thanks for video, my brother is suffering from IIH, though he has no signs as he is in the early 40s, and weight is decreased to the high 50s. His papplidum has been decreased may be due to dimox, but field visual test never always shows some problems. MRI is always normal, We consulted various neurologists, but they are suggesting for VP shunt operation, which my brother wants to avoid as there is no guarantee. He had 3 shunt operations when he was in his teens. We are only worried about gulocomma. Is there any m3dic8ne to recover from glucomma ?

Thank you for the information -however it is somewhat a misconception that weight loss will "cure" IIH - Treatments vary -some with a stenosis may need a stent as more research is coming out with that.

Neha Bhatia papilledema is a sign of increase ICP but LP still valid (not contra-indicated) as I think

High ICP can be caused by a space occupying leasion (risk of herniation with LP) or be idiopathic (no risk of herniation, LP can actually help with symptoms). So LP is contraindicated in papilledema until brain imaging has been done to rule out a CSF obstruction.

Dr. Papilledema is one of the manifestations of increased ICP, and Lumbar puncture is a contraindication in that case. So, how does it work, can you explain? Thanks
