Grading:
WHO grade 1: less than 2 mitoses / 10 HPF or Ki67 index less than 3%
WHO grade 2: 2 - 20 mitoses / 10 HPF or Ki67 index 3 - 20%
WHO grade 3: mitoses more than 20 HPF or Ki67 more than 20%
2018 WHO changes - NET G3 vs NEC
NET G3 = NET morphology retained and mitoses more than 20 HPF or Ki67 more than 20%
IHC: synaptophysin, chromogranin

I found it in appendix after removing acute appendicitis....so I need more surgery to remove some part of big intestine? My doctor is saying need to remove some part of intestine 😔 please advise me

Hi Prashant, a treatment is dependent on proper grading and staging.

If an appendix perforated with neuroendocrine tumor, would that be a cause of worry for spread?

Perforation of the peritoneum by the tumor or direct invasion of other adjacent organs or structures equals to pT4 stage, which is associated with higher risk of metastatic spread and lower survival rates compare to lower stages of the tumor of the same grade.

@@FilipSokol Thank you SO much for a response! I do have one follow up question. If the tumor itself did not perforate, but the appendix did, where a neuroendocrine tumor was present, would that be a cause for concern? For this instance, the tumor is 4mm/with muscularis propria invasion (G1). Would the fact that the appendix perforated with the neuroendocrine tumor present (tumor intact), automatically make it a pT4?

I have grade 1 neuroendocrine in my appendix.Hopefully no spread to the lymph nodes.Drs want to remove part of my colon anyway.😞
