Thanks so much, this is absolutely what my PT says ... it's a nerve theme, not a muscle theme ... so she forces me to IMAGINE the movement and then to work on performing it with the body. AND IT WORKS. Slowly, sometimes ridiculous and in tiny steps. But you both are right, it develops and the body relearns. Many many thanks to you both 🧡🙏🧡.

This is next level knowledge. I was shot in the head almost 6 years ago. And I've been inefficiently toiling towards regaining movement for the past 5 years focusing strictly on building specific muscles and not looking at my rehabilitation in this way. This doctor really has passion for what she does and she is THE-BEST-PHYSICAL-THERAPIST-ON-THE-INTERNET.

Video is hree months old and I am late on comments that Tara asked for. My stroke was 2016 with left side issues. Bound and determined to walk and found myself with all the spasticity issues Tara covers. I gnored them all and walked funny all over town until I had a tooth abscess. Fought my way back with repetition. My brain fought me up until the time the persistance regained brain motor control. The key as stated is to never quit, be persistent, stay positive, and evaluate your own performance daily and make corrections.

I found your videos while searching for a way to strengthen my hip flexor and psoas so I could improve my gait (incomplete SCI.) After watching a dozen or so of your videos I have redirected my home therapy. Thank you ! Now, I'm super motivated and inspired by your direction! Thank you so much for your videos. I was diagnosed with a stroke 4 months ago 32y/o and followed your vids which helped me a lot. however now my diagnosis has changed after a repeated mri to Functional Neurological Disorder (FND). This video in particular is so important for patients with fnd as no one seems to know much about it and this is literally what you have to do- teaching the brain new skills to do automatic movements. If you have any other tips / exercises for fnd honestly so many lost people would appreciate it .

Very well explained! You are a god send😃 I got all excited about the skate board and immediately tried it. My leg alignment was much smoother and less impact than actually walking. Also felt the benefit with the hamstrings. Thank you! You are a gift.

Is not about strength but about control, hence quality over quantity...37 yr old. Stroke survivor here and 4 months in recovery. I can walk with a cane somewhat. And arm can extend 90 degrees outward. Working on hand release currently which is right up my alley hehe. Thanks for all the great vids doc!

I saw in your Facebook stroke survivor's group, someone had posted a question about when he sneezes his affected arm flexes up and you told him to "strengthen" the muscle in opposite of the muscle that creates that movement! So that's why I made that statement!

Travis, the same happens to me; I ws affected left side; and when I sneeze or cough, the left leg and arm jump up and react with every cough or sneeze! I say to myself.... Wish I could move that waywhen I'm not coughing! Its strange to me to say the least... just mentioning because of your statement...have a great day!!

I’m glad I came across this video. I’ve been watching your videos for about 6 months and I’ve learned that you had this as a value. I have spent most of my life with a decent focus on strength building (+ aerobic, etc.). I still do some of the strength-building exercises I had done before the stroke (I had switched from weights to resistance bands a few years before the stroke). But, I have made the adjustment by adding a number of exercises/movements from various videos. 😊

As somebody on the outside trying to further understand rehabilitation, I see it as an area that contains different forms of rehabilitation depending on a patients diagnosis. To put this as simple as possible rehabilitation can be used to re-teach a person muscular strength, muscular endurance, neuromuscular control, and range of motion. Of course these four main areas can be built upon in order to give patients the best treatment possible for there diagnosis.

Hi I AM NEWLY subscribed to your channel having had a stroke a year ago and have found this particular clip really helpful. It makes sense that movement and rehab comes back to my brain and creating / strengthening pathways that will with continued practice help me relearn and re-gain as normal as possible strength and movement. Hi Dr My problem is Spinal compression T10 L4 L5. I lost my legs for 10 Mths in 2015. After spending nearly all of 2015 in bed. I'm standing and walking with the frame at the moment and trying to catch up with your videos. I'm an X athlete - I know all about strength speed and power, what you said is absolutely right. it's the journey not just the destination and you need to get into the driving seat to drive with right experiential learning processes and not necessarily with biggest engine, I don't know why but i feel melancholy at the moment. To strengthen or not to strengthen a particular muscle confuses me. While in therapy my PT pointed out how weak my hamstrings are and gave me exercises to strengthen. They also had me work with weights. My current goal is not to strengthen but to be able to walk more than 20 minutes without experiencing the debilitating glute pain I experience now. I exhausted my insurance allotment of PT so I joined a local adaptive gym where I work with a private trainer. We think the glute pain results from quads/hamstrings not firing resulting in glutes doing all the weight bearing when I walk, so we're working on that, which includes some strengthening work. BTW, your videos bless and inspire me. You are making a difference in our lives. Thank you Dr. Tobias! One last point: I experience the debilitating glute pain when walking with a cane. When pushing a grocery cart at the market I experience zero discomfort, even if walking an hour. I wonder why. Maybe a posture issue?

Thank you for watching and for commenting! I agree that this might be a posture issue. Problem with traditional strengthening after a stroke is that if you do traditional glute exercises, you will inevitably get better at that specific movement (for example: improve your movement doing a squat or a bridge). This will not necessarily translate to "functional movement". I meant to mention this in the video but I do not think I did. Check out the hip/walking playlist. You might find some "movement retraining" activities that will also strengthen the glutes :) I hope this makes sense!

You are correct. It is a long ride. But the one thing I love about this community is every single person who is taking the time to watch these videos really wants to get better. That being said, wanting to get better and having the discipline to put these videos into practice is 90% of what is required to get better. Hang in there!! You can do this. One day at a time.

I slept on evening, i had no problem, but i took three pils of lonarid (paracetamol+codeine), In the morning i had a stroke, during the night on my sleep. Drop foot with left hand paralysis were the symptoms. I HAD a serious impovement since than, but this video of yours made me to understand what you doctors say about neuroplasticity of the BRAIN

I don’t think these videos focus on muscle strength and that is why they work! I think you do an excellent of emphasizing neuroplasticity and it’s importance in recovery from a stroke.

I focus a lot on strength training, stretching and improving my gait as I also have moderate to severe RA.

My five year post-stroke teaches me that early stages of your recovery, you illicit every muscle to accomplish required movement. But after that you start isolating the necessary muscle and fine tune it. Then it becomes part of your autonomic function and uses very little effort .

I am interested in trying toregain the ability to move my affected hand and arm and exercises to do towards achieving that end: also correcting my foot’s supination: since I can no longer afford therapy: your tapes are so very helpful re things I can work on to correct the sequel ae of my stroke am further ahead with moving my leg (stiffly) thanks so much for your you tube series😬🙏🙏👍🏼👏🏼👏🏼

Dear Dr Tara, I am a survivor of brain haemorrhage of 26 years plus. My right side is affected & hopefully you can show me the exercises to do so as to move as swiftly as possible. And in fact, did try to cycle last year but just fall down. In fact, I don't seems to have any sensation when grabbing :(

How is it so hard to understand that for every muscle to contract, another muscle must relax. I have flexor spasticity due to multiple sclerosis and a fractured hip. I have begun weight bearing using an old sandal on my right foot. The Coremax device is so useful as is an the Razor scooter skateboard I found at the dumpster. "Walking" down the stairs has a whole new application.

I love your videos. I have learned SO MUCH from them. I'm working with a stroke survivor, supplementing his PT exercises and have found your exercises so helpful. He has left side hemiplegia with an inverted pattern, like so often you describe. He was a big time gym rat before his stroke and had wanted to add that back into his routine as soon as he was discharged from rehab hospital. I ask him to watch your videos before he have a rehab session to remind him that he needs to think of small movement patterns and we are getting those, albeit tiny at this point (external rotation at shoulder, etc.). My question is, should he still work out? I see no problem with him using the some of the cardio machines but should he not do strength training at all at this stage of recovery or should it be modified? I have encouraged him to work out bilaterally on weight machines when possible and use light loads in a 20 rep range, just so that he doesn't feel like he's losing all his previous muscle. But I've cautioned him about overuse of his non-effected side, especially with heavier loads. I definitely encourage him to spend more time doing the "micro-movement" exercises such as you and his PT show more than time doing regular workouts at the gym. Do you have any cautions about strength training in conjunction with the motor pattern training you do? Thanks!

I can understand why people use these videos to strengthen muscles- I often look for ways to do this myself, as I feel I am misusing my left hip, hamstring and quads due to the knock on effect of abnormal foot movement.

Hi, my husband spasticity is severely impacted and due to his 7 strokes, he cannot move his hands. He has strength but the messages from the brain movement is not working fast enough . Any suggestions? Thanks so much

Hi I had a stroke a year ago, my hand and fingers move, there is a little spasticity because I missed a lot of time (6m) at the beginning until I started doing rehabilitation normally (long depression😢) now all the fingers move at the same time (bad synergy?). my little finger bends in a spasm in the middle of the palm, I have never encountered such a problem anywhere on all the sources for a stroke, but I don't know how to overcome it. Is there still a chance to learn to use my hand/fingers normally again at this late stage

Thank you for commenting! The fingers are tough. I am currently working with an orthotist to design a wrist and hand brace. The best thing you can do is brace if it has a tendency to always stay in a ball. Unfortunately, in 20 years, I have not found one I love so I am working on designing one that I plan to have ready for sale end of this year. i had a mild stroke last 2017...the left part of my body was affected... im a musician and i cant play the piano and bass guitar the way i used to play before...my left hand and fingers are weak that they cant even move fast..very slow reflex...what arms hand and fingers exercises would you advise to regain the strength of my left legs arms hand and fingers? i want to play piano and bass guitar again..so that i can have my job back...i hope that i can get advise regarding my problem...thank you so much...God bless...

Everything you said is very right! It gives me hope. I do a lot of exercises and routines but the truth is that my walking slowly deteriorates. Do you think that PT may help reducing the effects of MS?

Hi "G G"! Thank you for watching and for sharing your story. (first, I must preface this with the fact that I am slightly biased). That being said, I think with a diagnosis of MS, you should always have a PT on board. Whether you see them 1x/week, 1x/month, or you have their number if you suffer a set back or have an exacerbation, it is good to have someone who knows your "movement history". They will have the best data to know how to modify, or add to your home exercise program when and if necessary. Hopefully you can establish a relationship like this with a PT in your area.

My sister has been diagnosed with neuromuscular disease. Basically, her finger muscles are not strong, she cannot gain grip with her fingers, cannot even try to press soft rubber ball with her hands. Same issue with her leg part below ankle. Any stretching and strengthening exercises for her pls?

Thanks for watching and for this excellent question! this is a very common problem. I suggest going back to the company that made the brace to see if they can get it to control your ankle better. Also check out the spasticity playlist. You will find some great exercises in that playlist.

Dr Tara, can you record for us what exercises you are doing with the patient in the first, second, third class? How do you combine exercises? What does it look like to work with a patient who comes to you? From where do you start, with wich exercises? If you have a patient in a wheelchair who is unable to stand up on his own, but can make steps himself with the cane, of course relying more on the stick instead on his healthy leg, in what order do you exercise? If the arm is completely motionless. Whether one day you focus on the leg, leg movement and walking and the other day only on the arm or you combine it all in one go every day. In a sea of wonderful and more than useful exercises here on your chanel, I don't know how to combine them and from where to start while helping my mom who had a stroke.
