This is one of the cool things about the journey and his videos. There are so many things that tie into each other. The joker smile or "fake smile" on his face shows up on the album Therapy Session. It comes from NF Real music and him rapping about his life no matter how bad or hard and people saying he should smile more. It's his way of saying, here is this better. It also is kind of like the persona he puts on to hide from the pain, "is the me or the Fear talking".

The painted on smile at the end symbolizes that even though he made it, he still isn't really happy. One of the kids in the basement makes another appearance in Motto. NF is a genius.
