Non-Inferiority trials look FAR too easy to manipulate. With all the problems we already see in the current research world, for me it's a killer problem - a red flag that basically only says "we got nothin' but we have to make it look like we have something". Only if there is a _significant_ advantage in price, tolerance or convenience should this red flag be ignored.

if you pick a pretty tight margin, then they can be effective even with relatively marginal benefits, but drug companies never do, because that threatens to undermine their bottom line
