whats the main differences between this and the regular one? and is it worth it jn your opinion?

The main difference is the forearm pocket. If you have no need for that extra storage, I’d go for the original combat shirt. The reason I got this one was because they were out of stock of the original in the colors I wanted. This is a fantastic quality top and I highly recommend either version.
