She contaminates her left hand at 2 minutes in by touching the non-sterile outside of the saline bottle, and then she uses that same hand to reach into the sterile kit and touch various supplies.

She did not, the cannula is the only part that is sterile and she had a disposable one so sterile technique not necessary. Cleaning is not done sterile technique in either situation.

Nothing is truly sterile, get over yourselves. Plus in real life you're working home health and doing this in someone's nasty trailer trying not to let their dogs, cats, kids crawl over your "sterile feild". You do the best you can.

Thanks! And I also thought a person could put the new trach ties under their neck before doing the sterile care. That way you won't have to try to hold their trach down while trying to thread it under their neck, but it was a great video review and I really needed to have this good review.

I think she also broke sterility after she pulled the wet gauze out from under the flange for cleaning. She touches the contaminated wet gauze with both hands and then uses those same contaminated hands to pick up another sterile gauze and uses her contaminated hands to ring the excess sterile water now contaminated into the sterile field.

Its best practice to secure the new tie before removing the old tie, especially if you are doing the procedure alone.

Great video. I always use a helper to hold the trach while I replace the holder velcro straps. Reason being, I usually cut them shorter after sizing them up on the patient. On a separate note, I would never allow them to put a trach in me. Once someone has a trach, its down hills from there. After a year or two, almost always they become MRSA and/or MDRO. Constant infections with trachs. They are unnatural.

I am private duty for a pt for the last 6 years, they have never had any type of infection. Once or twice the stoma became irritated but with appropriate interventions it healed quick. The type of trach care I’m seeing others do is not appropriate for long term management. And nurses rarely perform trach care in hospitals.

In our class we give the patient oxygen as soon as we take away the inner cannula.

They were saving a glove change by removing the inner cannula first. You could remove the inner cannula just before changing gloves and inserting the new inner cannula.
