1 of the biggest problems I'm seeing when new nurses r precepting is, they r focused on getting task done b4 their shift is over. If they feel like they're getting behind, they panic & get flustered. That is not the type of experience a new nurse should b around. All she had 2 do was go thru ur checklist & use that as a guide. They r not confident n teaching. I like 2 eyeball my patient b4 I take report. That way I know they're alive & what they look like. Then I either look @ orders or do my initial assessment. There's no wrong way 2 do it. The 1st thing I would let a new nurse know is we will b here past 7:30 to go over documentation. That way, they understand how important documentation is, & how important it is get specific things done on time. Whatever I don't know, we'll figure it out & learn 2gether. Nights is a different beast. People pretend nights is easy. It's not fast pace, but u will run around all night if u get the right patients, esp when those admissions come n during ur med pass.

I travel & am finding they're not making assignments by acuity. I find that 2 b crazy & dangerous. I try 2 support nurses that experience what u've been thru. I feel bad when my contract is over & I have 2 leave them. They try 2 give me the worse patients, but I'm built 4 it, so it doesn't phase me. That doesn't make it right either. I never felt like my license was @ risk bc of the way I document.

So how do you document when you have a heavy assignment? I'm thinking about leaving bedside because of the increase patient load. I get to my location 45 min-1 hr early and start looking up orders at the 30 minute mark if pt assignments are available. I've found it harder to do my job and care for my patients like I want these days. With not having techs (or when there are techs they don't really do their job) and no real teamwork. I'm running around crazy and most are just chilling at the desk. So I'm trying to be adaptable with my current method of doing things. I know for a fact some of these nurses aren't physically doing assessments... but are charting they did 🤐.
@mistermilkman
@mistermilkman Жыл бұрын
@@HeavenlyFaithful I usually ask what is required my 1st day on the floor. Whoever precepts me 4 that day, I'll write down what is required as we go thru documentation. I keep a folder & organize it based on things important 2 the floor. I come n 15 minutes early 2 get my assignment. I don't come n extra early bc assignments change last minute. I don't like wasting my time. I go 2 each room & eyeball my patients & let them know who I am & that I will b back 2 assess them after I get report. When I'm done with report, I do a quick look up, meds & all. I do my assessments & address any concerns. I focus on documentation after I do my evening meds. I may squeeze 1 or 2 n after I do my physical assessments. Depending on the day, I don't finish my documentation until I'm done with hand off 4 next shift. I've seen nurses complete assessments b4 they even look @ patients. I can only document what I observed myself, which is y I don't leave notes. Most charting systems contain areas 4 u 2 input ur findings & any care provided. It keeps me from having 2 explain anything extra. If nothing significant happened, keep it moving. Over-documenting can hurt u.
@mistermilkman
@mistermilkman Жыл бұрын
@@HeavenlyFaithful When my assignment is heavy, I input total assessments & only what is pertinent 2 the patient. I cut out the extra stuff that may b repetitive. That's when knowing the charting system is important. I also only chart the necessities on high acuity patients. If the patient is walkie talkie or require only set up assistance, I may skip portions of ADLs. If a patient has wounds, I address the wound/dressing appearance or if I changed the dressing. I focus my documentation on what's important n the plan of care of the patient. I don't think twice about it. U can also document according 2 the orders placed n ur patient. B mindful 2 overlook old orders. They can b confusing & have u doing unnecessary work. I also document n the patient's room as I do task the aide is supposed 2 do. I'll roll my WOW as I provide care & document output or vitals as I do them. Sometimes u have 2 switch up ur routine. Let them do what they do. Only doc what u c.

Ayesatu. I worked in aged care as a ND AIN. (Assistant in Nursing.) across three different aged care places. (I did my education in the height of the pandemic and as a result? Only got one week (40 hours) of Vocational placement at the end of the course... instead of 3 weeks (120 hours.) The place that I (along with 4 other students.) were sent to to complete that week? was within an utterly appalling state work culture wise. I swear, If I had gotten the full 3 weeks of placement? I never would have proceeded to get into the industry at all. My first two AIN aged care roles (doing afternoons/evening shifts.) were literally hell (bullying from co workers, management etc.) on top of co workers not wanting to mentor me. (Luckily I am fully switched on and never ever did unsafe practices at any time regardless of backlash from the lot of them.) Then I thought I finally made a breakthrough at a facility literally 2 minutes walk from my house. (I saw it being built and knew what was there prior etc.) Yet One RN had a real go at me on my first training shift.. I put her firmly in her place and told her to go shock some sense into herself with the nearest Defib.. and refused to work with her for future shifts (which the management agreed to.) (This RN ended up suffered a heart attack after a night shift ((15 minutes after they got home from work.)) They started having symptoms on site throughout the previous night but heavily verbally abused every single member of their team who were concerned for them and were swiftly attempting to organise paramedics to get them to hospital for help. I don't know the outcome. (they left the company in the end.) I usually do not wish death upon people... But I did for them. No one liked them. All was going well until I suffered from a partial dislocation of my left shoulder (caused by underlying bursitis as a direct result from manual handling strain in regards to heavy/dead weight residents during rounds.) The injury happened at home.. but it was ruled by my physio and my GP that it was all work related. Attempted to get onto work cover for financial compensation --- It fell through. That was the last straw. I had had enough of it all. I resigned and it was a bittersweet reality. The management fully understood and wished me the best. I miss the residents and some of the workers who I made proper connections with..... but... It is not worth getting more injured.. How am I to care for people... if the system in place does not care for the carers? I can't.. and I won't. After all that hard work and effort I put in... all that crap I copped... It frustrates and angers me that it did not work out. But now? I see that it is not worth my physical, mental and emotional states of being. I don't regret it really.. I now have the skills I need to provide care to my parents when they get old (as I am an only child.) to keep them out of residential aged care for as long as possible. I'd rather have them with me and I care for them... than to ever subject them to the flawed and flatlining system that is filled with failures. I actually wanted to get into the hospital sector (I hold a cert 3 in heath services assistance.) but due to lack of experience? No hospital (even the one I have been in as a patient for more times than I'd like to count in the last 5+ years.) would give me an ounce of a chance. My cousin is an RN and she gave me ample amounts of advice.. yet I never did get to interview stage.(I got one offer... in my second aged care role... but my parents wanted me to stay in said job I already got instead of suddenly switching routines and such... I do regret not taking it. -_-, Yet hospitals aren't that much better.. so... Who knows where that would have led if I did take it. Ironic isn't it? That healthcare... is riddled with unhealthy people.. and not the ones we are trained to take care of.... Cancerous co worker and management natures will continue to metastasize until the sectors snap under the strain of it all..... unless we de tox the workforce and weed out such workers who cause such systemic decay. ... How to do so? Your guess is as good as mine. -~-, I am now set to study Photography and I am working on learning 3D Printing~ ❤ I would rather work for myself than ever work with anyone else in any other sector ever again. Good on you for prioritising your health girl! All the best for the future!
