Hi there! You are so welcome :) Heparin may be used in conjunction with tPA therapy, but this is patient-specific and totally depends on the overall patient condition and diagnosis. It is not always given with tPA. Since these medications can be high-risk for the patient, always be sure to follow orders and question any that you suspect may compromise the safety of your patient!

You may adress it but i had a right MCA ischemic stroke i was given TPA. But they still transported me by ambulance about 100 miles for a thrombeectomy. Why wouldn't the TPA have been enough?

only confused about the end comment that tpa should be administered after 3-4.5 hours of onset. I am being advised in Nursing School that patients who are admitted to the ER and say that their symptoms could have started more than 4.5 hours are NOT eligible for tpa. Is this not correct??

Thank you so much for your question! TPA should be administered within 3-4.5 hours of onset, so anything beyond 4.5 hours of onset would be beyond the acceptable time frame to start tPA. I hope this helps provide clarity on this topic!
