Hello!! I'm a first year nursing student and I found this really helpful for making my case studies. I have a question to ask. Do all of the ndx (health promotion, risk, and problem focused) should be connected to one another? For example, I have made a health promotion diagnosis stating about " readiness for enhanced coping" and then for my risk is "risk for impaired skin integrity", and for my problem focused "acute pain related to movement of bone fragments as evidenced by mask of pain". Please help, I'm having confusions for this one.

if the goal has not been met, or partially met, do you do the the whole care plan again starting from assessment?

I guess you can say that. I mean reassessing your patient is part of your evaluation too. And based from your evaluation, you are going to modify/add certain interventions and the whole process starts once again until you achieve your goal. Hope this helps!
