As an ER nurse, we were wholly under-valued here. We deal with absolutely everything, we keep people breathing until they can get to ICU, and we have no rapid response. We ARE the rapid response.

I love what you said about hospice: how it’s possible to have a beautiful passing. My dad died last year and he was in tons of pain but, before he slipped into the terminal delirium, told me and the hospice team that he absolutely was not ok with the SQ pain pump and absolutely was not ok with midazolam at any point. He wanted to be awake in his body for as long as he could. He had a picc so, since I am a registered nurse in this state with specialty in med surg, the hospice team granted me delegation to manage his pain that way. It was about 380 hours until he passed where I took naps during those several hours before he needed another round of pain and anxiety med pushes (with the help of his friends). I would have done anything for him and was probably so hyper alert due to the circumstances that I was never feeling off my game or sleepy. Anyways, he had told me 7 years prior that he wanted to die in his home, the one he built himself and in the same bed his wife and love of his life had passed the same year we discussed his wishes for when that time came. They discharged him from the hospital stating that it was unlikely he would make it through the night. He passed 16 days later after I had dozed off for about 20 min next to him in bed holding his hand. Before that nap, I had given him a huge hug and told him how lucky I was to have had him as a dad and that he deserved all the peace, comfort and joy in the world. During those 16 days, 2 of his best friends stayed at the house along with myself and my 2.5 yr old daughter, the two of us already living 1:31:06 there during the palliative times. Every. Single. Day. Was. A. Celebration. Of. His. Magical. Life!!!! Even as he spiraled with the delirium, we would just follow along with whatever odd and irrelevant things he would say then turn them into parties or games. It was so so beautiful. It was silly and we would laugh until we cried… even if those tears were really the pain of anticipatory grief, still the most inspiring way to witness someone transition out of their body. Still crushed to this day but those memories are a warm blanket for my grief.

I feel like with hospice dealing with the death is easier because when they’re coming into your care everyone knows what the outcome will be. Even in cases where something traumatic happened to the patient and they’re dying “out of life order,” once the family decides on hospice care there is goal to “look forward to.” (AKA death with dignity and as comfortable as possible)

Sometimes yes but there are still time when people are in the denial phase or the family got a sudden diagnoses they weren't expecting. Like the person went to er bc they jaundice and found out cancer that's metastasis throughout the body and brain. It's difficult for the to process. But for the most part hospice is nice bc your bread and butter is in the psychosocial aspect and helping through the grieving process.

I thought that would be the case but unfortunately a lot of people and families choose hospice and don’t fully understand it, I get accused constantly by family members who still want heroic measures and accuse you of not doing enough and even trying to kill them. Gets tiring when you’re giving 💯 or more sometimes. And some of them are on hospice for a while and that can complicate things with the family wanting more.

I take exception to your opinion about ICU. Yes, I’m very attentive to details. But, the greatest gift is helping to save a life. While we don’t talk to the patients a lot, we do talk to the families. There’s nothing like reassuring a family. Or, helping and understanding a family when it’s not going well. ICU has taught me how wonderfully made our bodies are made. And, it made me grow up emotionally. Sometimes I cry, and it’s ok. I’m an empath. And I’ve made my peace with that. I tried to work in other areas. But working in an ICU in a level one trauma center is my happy place. The good thing about nursing, is there’s always somewhere else to go.

My mom had nothing but great things to say about the ICU nurse who took care of her dying husband. He had a fall and it was of course, unexpected that it was fatal. The nurse was very straight with my mom about his chances, and provided comfort. Thank you for your hard work and compassion! 🎉

I love this! Peds nurse here with experience inpatient med/surg, outpatient peds pulm, school nursing, PPEC, and now outpatient primary care peds while I work on my PNP! School nursing was so sweet but sooo boring and a lot of paperwork. Yes you are the only medical professional in the building which is weird. I did a lot of trainings with staff at my 4 schools on anaphylaxis, seizures, diabetes. I loved the education but the pay was terrible. Hours were amazing - 7-2:30 school days only. One of my favorite things was field days! And interviewing my little patients without their parents…. Interestingly one of the only times you will talk to kids in peds without parents around. The stories are great!

@@YeshuaKingMessiah My situation was a little different. I worked for the department of health and oversaw 4 schools so I was a state employee, not a school district employee. Benefits were amazing but the pay was embarrassing... I think I made about 28k a year. But I had all school breaks (including summers) off so that was really nice esp for parents.

Did Burn ICU for 10 years and loved it, even though everyone said I’d hate it. No revolving healthcare door of sickness - patients get burned and mostly get better and go home, or we know initially they won’t survive their injuries, so not a lot of surprise deaths/codes. Because most patients get better there is lots of job satisfaction that I never got on the chronic cardiac/medicine/cancer units. Covid ruined ICU/nursing so made the change to VIR (Vascular Interventional Radiology) this year - which is what I consider single serving patients. Patient, docs and staff all super nice. The patients (adult and peds) are usually really grateful and they all come and go, not much family involvement or drama, no poop and everybody gets to go home. If not they get admitted, but everyone gets to leave our unit. 😁 Clinic work/hours for hospital money. Plus no weekends (unless you want), closed on holidays, and some on call. 💯 totally worth it.

@@trying3058 VIR = Vascular Interventional Radiology. Basically it’s image (fluoroscopy/CT/xray/ultrasound) guided Neuro (angiograms/stroke embolizations), vascular (thrombectomies, fistulagrams) and biopsy (liver, lung, kidney) type of out patient procedures. It’s VERY interesting! Plus, at my hospital, because it’s procedure/operating room based it’s a 20% raise over floor/ICU nursing. Budget is never a problem, we are always staffed, and we never float! 👍👍👍

@@trying3058 there are 2 (RN) lanes in VIR: prep/recovery (no lead) and procedure (lead). The prep part involves starting the IV, giving premeds, checking vitals and meds, printing consents, and the recovery involves checking post op vital signs, monitoring for post operative complications and discharge teaching - no lead. I work 4 days, 10 hour shifts, so I do both prep and recovery. Some nurses only work prep and recovery, and they don’t wear lead at all.

My logic is, if no one would become a farm worker, eating as we know it would be very difficult. Every specialty should be appreciated and paid equally. It’s simply a choice of patient population and environment.

OR Nurse here. Watching on replay and enjoying it greatly. Perioperative nursing is very orderly, with a highly regimented way of doing things. Boring? Sometimes. Until a Level I trauma comes in. Then it becomes fairly chaotic, with blood loss/transfusions, etc. It can get very messy; patients sometimes are more apt to have 'evacuation issues' when they are completely relaxed. 😆Surgeons are a mixed bag, but if you have proven your competency you may have a little leeway in how you respond to them. Patients have a wide variety of conditions ranging from the trivial to the ridiculous. Wouldn't trade it for any other specialty.

I work in a Peds Level 1 Trauma OR, and my back hurts! Haha, we move the OR beds, C-Arm machines, heavy pans of instruments, moving 250+lbs teenage boys back to the patient bed after surgery (Peds can have adult size patients too!) This is a great video!!

Great minds. Think alike. Psych APN here/ left private practice for bedside due to money driven systems with no support- ER many years and in many ways so much easier- I feel your opinion on education in nursing deep down in my soul! I have two masters- love students- cannot do the “mean girl” thing! I gave up a full scholarship for doctorate in first semester because I’m too old for that mess! Keep up the good work nurse Liz!

Have only worked in one specialty but I’m convinced it’s the best place in a hospital and it’s not listed here. It’s postpartum 💕 not something that can be it’s own field for providers, but it can be it’s own field for bedside RNs if you are at a large hospital

New grad here (taking my NCLEX next week aaaah!!) and I was offered a job in a level IV NICU. I am very excited about it. - I am hot natured and always sweating when people are complaining about freezing so i’m not excited about how warm the NICU can be. However, when I shadowed I was actually a little chilly at times, to the point of needing to wear my scrub jacket 👀. I attribute this to being because I didn’t have to run all over the unit like a wild animal. I am sure that there will be times that I do have to do that, but at least I know it isn’t always 🤣

ICU patients are usually sedated for their own safety and to prevent medical PTSD.. being awake with all of the monitoring equipment, specialty lines and caths, and ET tube is traumatic. They are also very likely to fall or pull out a line if awake and delirious.

@@sunayya Not only do I insert PICC lines into patients, but I also insert midlines, insert ultrasound guided IVs , Access and deaccess chest ports and are consulted by physicians for best vascular access for patients of all different scenrios . A nursing job where both patients and nurses and even the docs are excited to see you, appreicate you and I get alot of doctors coming to our team for our advice.. I feel very respected in this position and proud of be apart of this team!

The only ones I could deal with was home health( one patient at a time for 1/2 hour or hour and then on to next one) and psych on night shift. I know my limitations lol 😂

Love ED but also you are right about inappropriate use of ED. As a PEDs ED nurse I can empathise with the overly anxious parents, overly anxious GPs who send patients in instead of managing themselves is really frustrating. As Scott says the best thing is that they tend to be turned over fairly fast and the ratios are 1:3 for us plus a supernumerary TL and resuc team to help out and ready access to doctors compared to ward work where you have to page doctors when needed One of the biggest nursing scopes of practice (for example we are the only nurses in the hospital allowed to cannulate as nurses, the wards they have to get doctors to do that) Almost no manual handling (at least the Peds ED) I can literally count how many patients I have had to move since I started here, compared with wards where my back hurt after every shift And lastly and best, NO SHOWERING!!! The non accidental injuries destroy me though, I had a trans boy who was so sweet to the point I found out he left a thank you not on his food tray every time. And then I am reading the doctors note that he had been sexually abused by his dad for years and the mum wouldn’t come be with him without the dad. I begged the doctor that we make sure this kid did not go back home, I was horrified when the ward nurse said they had sent him home but then he quickly clarified that he had been discharged to live with his sister so hopefully has a better life now.

I know some NPs who work part time in a hospital and do aesthetics. In OR, NPs can do injections and fillers. The only limits are they can't do any surgical procedures. It is medicine because you need medical training to paralyze facial muscles

Omg. I was cracking up re: inpatient Dialysis😂 My experience with Dialysis nurses is that they “appear “ to be from the No Touchy Touchy camp. Not saying all but yes, my patients would come back perkier, not feeling that great and looking like they’ve been thru a war

being a gamer yes the S stands for "Super" or "Special". Teir lists that gamers use came from the japanese culture of ranking things and so as people used the lists it just became commonplace to start with S being the best. In gaming S teirs are the one everybody wants or sought after as they are generally too powerful or "broken" in the game. AFAIK this is the time I seen a ranking system for nursing specialties and it is interesting. I am new to nursing but whenever I heard about what's "good" or jobs that are "bad" it's usually a one off here or there. Like somebody from ICU noted that dialysis nurses are pretty relaxed or somebody will tell me don't go to this specialty and why. I think this is a great way for new nurses to get a consensus of what their next moves after they get their new grad experience out of the way.

I would add Cruise Ship Nursing to tier A. You need the skills of Emergency, you do a mix of occupational health, lab work, XRay,,.. You see all age groups (except babies and the very elderly) and you get paid to see the world.

It is a wild situation in academia! I agree! I work in higher Ed, not in nursing education, but it is wild. Can be “mean girls” for sure. But also love the students and teaching. But publishing and tenure and the cliques….

As a hospice home health nurse it can be so rewarding helping someone pass how they want and provide comfort for their loved ones. The struggle is families not wanting to be involved so I spend more time with their loved ones then they do, elder neglect families who are not knowledgeable about hospice accusing you of trying to kill them faster. Tough

Have to share this, hospice nurse having a rough time burnt out. Almost 11 pm and a patients family just text to say how much they do appreciate my help. That goes a long way and helps make it all worth it. I know a lot of you will understand this❤

NICU S FOR SURE!!!!!! But as far as the parents being easier. ... not exactly a true statement. But it's so scary for them so you can understand the difficult days with parents. Our patients can be rather dramatic and cheeky never trust them... never.

It seems like long term care staffing levels have been steady over the last 30 yrs or so. The problem is that the residents today are not the same. Years ago, the care level needs were not anywhere near where they are now. Nowadays, the facilities have multiple residents who are receiving what used to be hospital level care. The good news is that people aren’t dying from as many ailments than from years ago. The bad news is the staffing levels have not increased proportionally to the level of skilled care need.

That one comment about oncology is so true, just had a patient receiving a $20,000 a month injection up until 2 days before he died and came onto hospice.

After a year and a half in the OR I came back to this video and everything you say about the OR is sooo true - except for the back part. Like any other nursing specialty, your joints will not be happy with you

Lol on the ER getting a D. I was an ER nurse for 16 years and agree with the earwax analogy 😅😅😅😅. They happen more than you know even in a level 1 trauma center. 😅😅😅😅 BUT that being said, ER gives you what no other unit can. So it doesn’t hurt to try it just to say you did it and survived. 😅😅 That being said, it’s understandable if some folks just don’t wanna bother and prefer their jobs to be super easy chill and predictable.

You're lucky this list doest have an SS rank. That will screw up everything. Mainly see this ranking in manga/anime lists of individual characters ranks in skills/techniques in comparison to others. The higher tiered you are the stronger and more capable you are. Most aspire to be A's or S tiered and SS is godly.

I loved it because the patients you get to know them and when they get close to death they and the family know they can count on you to keep them comfortable

ER is the best! Figuring out what is wrong with patients, no day is the same, never know what is coming in next, camaraderie with the docs, chaotic environment! Perfect for hyperactive and ADD personalities!

lol idk if you’ll ever see this but the rankings with S being top tier is an anime thing. Usually red is someone with a lot of power and green is seen as sweet/docile maybe even weak

S is for 'Special' yes, and it's red because for gamers, anything you put in 'red' is a 'must have' and 'must deny'. It's something that is so important to have in the game, that if you can't get it, it's critical to deny it to the other team.

@@RNurse787 don't change meds when you're told not to by the paitent cause you find it easier to manage. Don't harm paitent. Don't hit patients family members. Don't talk down to patient that knows more than you do especially about the specifics. Don't continue to remain and attend meetings when you are fired. Don't talk over paitent and family. Don't interrupt conversations you weren't invited to. I can go on. I pray they harmed themselves so they can no longer harm others like they did me.

As a former dialysis patient. You feel ok before treatment (maybe)...You feel like a zombie when you leave. As a future nurse... I couldn't work dialysis in a hospital. It's dark, gloomy, and feels morgue-ish to me.

I love your channel but this kinda feels like a closed group. as a life long patient I love healthcare it's so fascinating however in order to participate in this field your body has to physically function I think your channel would grow if you could include the chronic illness community we are just as interested I wanted to work in healthcare but knew I couldn't physically do the job . I watch your videos and find them so educational!❤️

I have some interviews and panels with patients who live with chronic conditions! I do mostly deal with healthcare topics since that's the field, and I'm not necessarily an expert with chronic conditions outside my scope of practice.
