CHAOS and EXIT Procedure: Baby Aydin's Story

Yadilsa Martin was 24 weeks pregnant when she and her husband, Jamaal, received a startling diagnosis: ultrasound scans performed at NYU Langone revealed that their baby had congenital high-airway obstruction syndrome (CHAOS), a rare and potentially fatal disorder that makes it impossible for a baby to breathe on their own at birth.
The treatment for CHAOS is a complex procedure called ex utero intrapartum treatment (EXIT). During the EXIT procedure, the baby is partially delivered through cesarean section. While still attached to the placenta, pediatric surgeons complete a tracheostomy to create an opening in the neck that allows oxygen to reach the lungs.
A team of more than 25 specialists from NYU Langone were in the delivery room on August 3, 2021, when Aydin Idris Martin was born. The EXIT procedure was followed by a series of complex interventions by specialists at Hassenfeld Children's Hospital at NYU Langone, including extracorporeal membrane oxygenation (ECMO) to take over the work of his heart and lungs after birth, surgery to correct a congenital heart defect, and a procedure to fix an anorectal malformation.
After months in the Neonatal Intensive Care Unit at Tisch Hospital and then a rehabilitation center, Aydin went home for the first time just weeks before his first birthday.
