Thanks for the video. Wheni uploaded mine on ODK and previewed. It does not show the options for multiple choices (e.g. male, female). All that appears is the "Gender" and blank circles to select for male or female. No labels

After following all the steps, this is the message am getting when uploading the excel form to online odk... Error: The survey sheet is either empty or missing important column headers.

Column headers are the column names. Check if you have included all the column headers/ names in all the worksheets. Also, only use lower case when writing the column headers eg name and make sure they are correctly spelled.
