Mr. Johnson a 68-year-old patient on the ward of a hospital where you work as a charge nurse.

Lives with wife , 2 daughters abroad .
Admission date : 20 March 2023
Discharge date : Today
Reason for admission :
An elective total knee replacement , difficulty mobilising and worsening of osteoarthritis.
Medical Background:
Diabetes 2012 and HTN 2016
Metformin and Amlodipine ( Noncompliant )
Nursing Management :
Daily dressing surgical incision site.
Range of motion , stretching exercise.
FBE and UEC alternative days.
Physio and occupational therapy .
Assessment :
Good mobility, good prognosis, weight - bearing with wheeelie - walker .
Continued observation . Dropped Hb postoperatively, requiring 3 units packed red blood cells, now stable ( 114 g/dl ) . Postoperatively disorientation during recovery, possibly related with anaesthesia.
Discharge Summary :
Monitor medication
Preserve skin integrity
Dressing every third day
Continue exercise , physio arranged for home . Wheelie - walker , wedge pillow and toilet raiser arranged from hospital .

Hi, what do you mean by "Was" ? Has he recovered from those medical conditions ? Has been diagnosed / he has the history of... is defferent from he was. Make sure you write accordingly. All the best 👍

Hi, There is no need of a comma before "who" as you are talking about the same person. You might have use comma after and before when you try to include more information between two sentenses. All the best👍

What about packed cell transfusion? Is that relevant for a nurse to write?is it ok to write his social history and past medical history to include in second paragraph ?
